The Investigation Of Rules On Syndromes And Medications Of Essential Hypertension

Objectives:To investigate the rules of TCM patterns and Medications about EH (Essential Hypertension), based on the analysis of datas collected from in-patients.Materials and Methods:This study had collected 400 cases of EH from the first hospital affiliated to Guangzhou University of Chinese Medicine, since January 2005 to January 2010, among which men were 154, while female 246 cases. All of these cases were qualified of the inclusion criteria and excluded standard. And all the original data was recorded in the Exce12003, and analyzed with pykno-statistical methods to investigate the rule of TCM patterns and Medications.Results:1.The rule of TCM patterns of EH:pattern of Phlegm-dampness＞pattern of Ascendant hyperactivity of liver) patternof yin deficiency with yang hyper >pattern of yin and yang deficiency. The rule of single TCM patterns:pattern of Ascendant hyperactivity of liver＞pattern of Phlegm-dampness＞patternof yin deficiency with yang hyper> pattern of yin and yang deficiency. As for the accompanied symptoms, syndrome of blood stasis was most, secondly was the syndrome of hepatic fire, and the qi and blood deficiency was least, which show that phlegm and dampness are the common pathological factors.2The overall rule of medication:The frequency of medication was:deficiency tonificatng medicinals, blood-activating and stasis-resolving medicinals, liver-pacifying and wind-extinguishing medicinals, cough-suppressing and panting-calming medicinals, heat-clearing medicinals, diuresis-inducing and dampness-draining medicinals, exterior-releasing medicinals, tranquillizing medicinals, wind-dampnessdispelling medicinals, qi-regulating medicinals. Commonly used traditional Chinese medicine was:Tianma, hoelen, Licorice, Uncaria, Achyranthes, Pinellia, abalone, red sage root, tangerine peel, Chuanxiong. And the common prescriptions were:decoction of Tianma and gambir plant, Pinelliae and Atractylodis Macrocephalae and Gastrodiae Decoction, Decoction for Clearing Away Gallbladder Heat, Powder for Restoring Pulse Beat, double solstice, Lycii and Chrysanthemi and Rehmanniae Bolus, the second prescription for coronary heart disease, Liver-Wind Suppressing Decoction, Rreinforce Yang and regulate blood decoction, two immortals decoction. The rule of medication and pattern of syndrome was:syndrome of blood stasis: achyranthis, danshen root, chuanxiong, herba leonuri, peach seed, safflower; overabundance of YANGf:gastrodia rhizome, gambir plant, abalone, oyster, tribuli, lumbricus, concha margaritifera usta;The pattern of phlegm:pinellia, pericarpium citri reticulatae; bambusae caulis im taeniam, arisaema cum bile, snakegourd fruit, the bulb of the fritillary, almond;Syndrome of deficiency of QI:licorice root, the rhizome of large-headed atractylodes, Dangshen, Shanyao, Huangqi, Taizishen; Ssyndrome of fire-heat: Huangqin, Chishao, Zhizi, Huanglian, Danpi, Maodongqing, Xuanshen, Xiakucao, Tian huafen;The syndrome of Ying deficiency: Maidong, Shengdi, Nvzhengzi, Hanliancao, Gouqizi, Tiandong, Guiban;The syndrome of Yang deficiency: Duzhong, Xianlingpi, Xianmao, Tusizi, Roucongrong, Bajitian;The syndrome of Blood deficiency:Baishao, Danggui, Shudi, Ejiao, Heshouwu.3. The rule of medication in different patterns:excessive rising of liver-YANG:Tianma, Gouteng, Shi jueming, Niuxi, Sangjisheng, Huangqin, Duzhong, Y ejiaoteng, Fuling. asthenic yin causing predominant yang:Gancao, Niuxi, Tianma, Fuling, Maidong, Gouteng, Danshen, Chishao, Wuweizi, S hengdi. The pattern of Phlegm-dampness:Banxia, Fuling, Chengpi, Tianma, Gancao, Baizhu, Danshen, Zhuru, Zhike.The pattern of deficiency of Ying and Yang: Sangjisheng, Nvzhengzi, Danshen, Xianmao, Duzhong, Yimucao, Hanliancao, Xianling pi. syndrome of blood stasis:Danshen, Chuanxiong, Gancao, Tianma, Fuling, Banxia, Chishao, Chengpi, Gou teng, Taoren. The pattern of liver fire:Tianma, Gouteng, Fuling, Niuxi, Shijueming, Banxia, Chengpi, Gancao, Zhuru. T he pattern of deficiency of Qi and Blood: Fuling, Banxia, Huangqi, Tianma, Chuanxiong, Baizhu, Gancao. 4. The decoction of Tianma and Gouteng would be the priority no matter what complicating disease happens. However for those abnormal blood fat without any clinic syndromes, there is no speciality. But for coronary heart disease, dissipating phlegm and promoting blood flow are important. For diabetes and cerbral infarction, medication for reinforcing deficiency is commonly used, and the medication for reinforcing Ying is more important for diabetes. There is no significant influence of course of disease for EH.Conclusions:1. The pathogenesis of EH is asthenia in origin and asthenia in superficiality, but in the south of the mountain, the pattern of phlegm and dampness is the most important factor of pathology. Besides, blood stasis is the common factor.2. The treatment of EH is treating superficiality in anxious station, and strengthening body resistance at the same time. The medication of Traditional Chinese Medicine is reinforcing deficiency, calming the liver Yang, promoting blood flow and dissipating phlegm, which correspond with the characteristic of pathology and therapeutic principles.3. For the pattern of excessive rising of liver-YANG and asthenic yin causing predominant yang, it's more important to strengthen the liver and kidney and calm liver yang. For the pattern of phlegm and dampness, it should be promoting blood flow and dissipating phlegm. If Ying and Yang are both deficient, it should be reinforcing deficiency.4. The decoction of Tianma and Gouteng would be the priority no matter what complicating disease happens.
