Objective:To reveal the present situation of the awareness of human papillomavirus(HPV) and HPV vaccine and the willingness of vaccination among urban and rural women, to analysis the age of initial sexual intercourse. To provide baseline information and scientific basis for the primary prevention of cervical cancer in the policy strategy to promote HPV vaccination in ChinaMethods:A multicenter cross-sectional epidemiological study has been conducted in Dalian city and Nantai District of Haicheng. A two-stage sampling methods was used to select our study subjects. A uniform questionnaire was applicated, informations were obtained by some rigorous trained investigator. Survey data using Visual FoxPro for double entry and management. Data was analyzed by SPSS 13.0 software, and descriptive analysis,χ2 trend test and Spearman correlation were used to test hypothese, testing standards for the a=0.05.Results:Totally 1200 copy of survey questionnaires were released, and 1160 copies of valid questionnaires, of which 660 were urban, rural 500, response rate was 96.7%.The awareness rate of HPV and HPV vaccine are lower in urban 17.6% and 8.0%, rural 17.2% and 10.6%; The knowlege that HPV infection may lead to even lower proportion of cervical cancer (9.2%,5.8%). Urban and rural women were only 3.8% and 2.0% of respondents aware of the appropriate HPV vaccine should be no sexual activity among women. Different level of education, to a certain extent, can affect women on HPV and HPV vaccine cognitive level, in which urban women showed higher level of education on HPV and HPV vaccine, the higher cognitive level (χ2 trend= 15.732, p= 0.000;χ2 trend=7.035, p=0.008), cognitive level of rural women with no education degree. HPV vaccination in the choice of institutions, both urban and rural women that the CDC and the Maternal and Child Health Hospital for the appropriate agencies. HPV vaccine on the immunization of urban and rural women age appropriate large differences of opinion. Performance for the urban women that HPV vaccination vaccine suitable age 16-18 years (24.1%),19-21 years old (21.8%) and three age groups 22-25 years (23.2%) the relative concentration of the views; rural women were 24.4% said "do not know" HPV vaccine in the appropriate age, the choice of other age groups were less than 20%.HPV vaccination of urban and rural women would have a higher proportion of vaccine were 68.2%,63.8%; willing to let her daughter vaccinated higher proportion of HPV vaccine (70.6%,73.8%). The main impact of vaccination will include survey of the age, education level, the vaccine's safety and efficacy of vaccines and vaccines do not understand and so expensive. Influence of age on vaccination will mainly in cities, younger HPV vaccine inoculation willing the higher the percentage, the difference was statistically significant (willing own vaccinationx2 trend=8.621, p=0.003; willing daughter vaccinatedχ2 trend= 5.219, p=0.022). Level of education on the HPV vaccine will also have an impact, showing the proportion willing to vaccination with the HPV vaccine increases the level of education (willing own vaccination:Cityχ2 trend=18.698, P=0.000, rural areasx2 trend= 11.949, P=0.001, willing daughter vaccination:City x2 trend=5.874, P= 0.015; ruralχ2 trend=5.152, P=0.023). HPV vaccine inoculation urban women do not want the main reasons were worried about the vaccine security (42.9%),the vaccine do not understand (31.0%),worried about the effectiveness of vaccination (28.6%); rural women do not want HPV vaccine inoculation major reasons were that:they did not suffer from the risk of cervical cancer (34.8%),the vaccine do not know (25.4%),the vaccine is expensive (23.2%). Urban women do not want her daughter HPV vaccine inoculation main reasons were:fear the safety of the vaccine (41.8%),the daughter of their own decisions (38.1%),worried about the effectiveness of the vaccine (28.9%); rural women do not want her daughter inoculated in the vaccine, the main causes:too young (42.7%),the vaccine do not know (26.0%),the daughter of their own decisions (19.8%).Urban women average age of first sexual intercourse,23.69±2.55 years, rural women average age of first sexual intercourse,22.63+2.28 years; survey of the younger age of first sexual intercourse, the smaller (rs= 0.289, p= 0.000) specific performance for the 55-59 age group 24.49 years old,45 to 49 age group was 23.45 years old,35 to 39 age group was 23.30 years old, 25 to 29 age group was 22.64 years old,20 to 24 age group was 20.17 years, suggesting that rural females show early age of first sexual trends.Support urban and rural women were HPV vaccine is the ratio of 61.2% and 63.0%, while there are 58.3% and 45.8% of urban and rural women suggest that the Government should be price controls and price subsidies. Urban and rural women's ability to pay on the HPV vaccine are low, urban women are willing to pay for imports and the amount of domestically produced vaccines were 600 yuan and 500 yuan,300 yuan of rural women, respectively, and 100.Conclusion:1,As for HPV and HPV Vaccine Awareness,both Urban and rural women was low, but high acceptance rate. The main factors that affect urban and rural women' receiveness of HPV vaccines are:the survey's age, educational level, the correct understanding of the vaccine, vaccine price, vaccine safety and long-term evidence of effectiveness.2,Urban and rural female adolescent age showed an early trend in the age of first sexual intercourse, suggesting that HPV vaccination should be appropriate to the age before 20 years old.3,Most respondents agree on the usage of HPV vaccines, HPV vaccine's promotion need our countries and govermnent's promotion and organizations, and also adequate health insurance coverage, health departments need to re-allocation of health resources...
