| Objective Study the manifest of meniscal injury between arthroscopy and magnetic resonance imaging(MRI), investigate the diagnostic value of MRI in meniscus injuries and its value for surgical indications.Methods We collected the data of 97 case include 114 pieces injury meniscus which diagnosed by MRI. Then the expression result statistics were analysed by SPSS 17.0. Use Kappa test analyze validity of MRI findings using arthroscopy as the gold standard, then analyze the difference between medial meniscus and lateral meniscus byχ2 test. According to the signal of MRI, there is divided into three groups, Group A:the meniscus of MRI showed the I level signal, group B:the meniscus of MRI showed the II level signal, Group C:the meniscus of MRI showed the III level signal. Analyze differences of three group in the number of injured meniscus which need surgical treatment by x2 test.Results 1. The sensitivity of MRI in diagnosis of meniscal tears was 76.9%(50/65), the specificity was 86.8%(66/76), and the accuracy is 82.3%(116/141). Our study showed good kappa agreement (0.58) for the effectiveness of MRI in diagnosis of meniscal tears. The effectiveness in diagnosing medial and lateral meniscus tears of MRI, there was no significant difference. 2. The only one case of 29 cases in group A was tore and treated by arthroscopy,14 cases of 52 cases in group B treated by arthroscopy. 50 cases of 60 cases of group C treated by arthroscopy. The cases treated by arthroscopy among three group have significant differences (χ2= 62.357, P<0.05). Comparison of each group, The cases treated by arthroscopy between group A and group B had no significant differences (x 2=5.332, P>0.02). The cases between group A and group C had significant differences (x 2=33.930, P<0.01). The cases between group B and group C had significant differences (x 2=47.781, P<0.01)Conclusions 1.MRI is a effective method in the diagnosis of the meniscus injury of the knee joint. Our study showed good validity of MRI findings and arthroscopy.2.MRI show a good reference for surgery. The meniscus of MRI showed theⅢlevel signal should treated by arthroscopy. The meniscus of MRI showed the I level signal and theⅡlevel signal can non-surgical treatment,we can treated by arthroscopy of no avail. However, clinical symptoms and signs should be taken into account. |
