| Objective1 To investigate ziprasidone and olanzapine on glucose and lipid metabolism in first-episode schizophrenia.2 To investigate the efficacy of ziprasidone and olanzapine in first-episode schizophrenia.Method53 schizophrenic patients who met Chinese classification of mental disorder were divided into olanzapine group(10-20mg/d) and ziprasidone group(80-160mg/d).Before and after 6-week treatment,some data which include height, weight, electrocardiogram(ECG),blood routine, liver and renal function, fasting blood glucose,fasting serum insulin, high density lipoprotein, low density lipoprotein,total cholesterol and triglyceride were measured. The PANSS and CGI-S were measured by the end of 2,4,6 week. The adverse event rating scale was used to assess the safety of the treatment.Results1 Compared with baseline,LDL,TC,TG,BMI and body weight but HDL,increased significantly after a 6-week treatment in olanzapine(P <0.05).While in ziprasidone, no significant changes were found. (P>0.05).Compared with ziprasidone group,the increasement of LDL,TC,BMI and weight were higher in olanzapine group and the difference was significant (P<0.05),but there were no significantly differences in the increasement of HDL and TG between two groups (P>0.05)2 The glucose metabolism indices before and after treatment:Compared with ziprasidone group,the FBG,INS and IRI were higher in olanzapine group and the difference was significant (P<0.05).The indices mentioned above were not significant change in ziprasidone group. The increasement of FBG and IRI were higher in olanzapine group than that in ziprasidone group and the difference was significant (P<0.05)3 There was a significant difference in PANSS score in each group before and after treatment (P<0.05).There was no significant difference between two groups in efficacy and side effects (P>0.05)Conclusions1 Olanzapine can lead to increase in low density lipoprotein,total cholesterol, triglyceride,fasting blood glucose,insulin and body weight after 6-week treatment.2 Ziprasidone does not affect significantly on the low-density lipoprotein, cholesterol, triglycerides, blood glucose, insulin and body weight after 6-week treatment.3 There are equivalent efficacy in ziprasidone group and olanzapine group after 6-week treatment. |
