Objective:To analyze the characteristics of clinical manifestation and serological examination of the newborn with ABO incompatibility, and explore the therapy for this disease.Methods:Clinical manifestation, result of serological examination, the method of treatment and prognosis of 56 cases of neonatal ABO hemolytic in our hospital from Nov.2009 to Jan.2010. were retrospectively analyzed.Results:All mothers had blood type O,23(41.1%) infants had blood type A(O-A), and 33 remaining infants(58.9%) had blood type B.25(44.6%) infants occurred in the first fetus. Jaundice appeared within the first 48 hours after birth in 46 cases(82.1%),57.1% of jaundice were not serious, but jaundice of 15 cases(26.8%) was severe (bilirubin. concentration>342μmol/L); 19 cases were associated with anemia which were not serious. Antibody release test was of the most important significance in diagnosis of ABO hemolytic disease, the positive rate of which was 100%, and the positive rate of the direct anti-globulin test was only 42.9%. Conventional therapy, such as phototherapy, fluid replacement and sodium phenobarbital for oral use, albumin and intravenous immunoglobulin were used as clinical indicated. The average time for Jaundice treatment is about 5 days.55 cases were Cured and one neonate quitted treatment.Conclusion:The main symptom of infants with ABO incompatibility is jaundice, while anemia may be an accompanying sign. The diagnosis on this disease should be based on antibody release test. The early use of intravenous immunoglobulin would have better therapeutic effect in some serious cases.
