ObjectiveTo evaluate the strengths and weakness of US and CTA on diagnosis of carotid artery stenosis and establish an optimized imaging examination program of cartid artery stenosis.MethodsFrom 2005-2009,65 suspected cases of carotid artery stenosis of our hospited were investigated. US and CTA were underwent one week before DSA in all patients. The result of DSA were considered as gold standard. The sensitivity, specificity and accuracy of US and CTA on diagnosis of carotid artery stenosis degree at less than 50%, 50-69% and no less than 70% were evaluated. The level of carotid artery stenosis was calculated with the method of ECST combined with CSI.ResultsThe sensitivity of CTA on the diagnosis of carotid artery stenosis degree at<50%, 50-69% and≥70% were 89.9%,90.6% and 92.6%, respectively. The specificity of those were 93.8%,94.9% and 98.3%, respectively. And the accuracy were 92.6%, 95.5% and 97.7%, respectively. The sensitivity of US on the diagnosis of carotid artery stenosis degree at<50%,50-69% and≥70% were 79.7%,69.8% and 59.3%, respectively. The specificity of those were 89.5%,94.0% and 97.4%, respectively. And the accuracy were 86.5%,88.5% and 93.5%,Meanwhile, there is a good consistency among US, CTA and DSA on carotid stenosis grading (Kappa1> 0.75> Kappa2> 0.70, P<0.01).ConclusionCTA and US can be considered as good assessment methods on the diagnosis of carotid artery stenosis. The results are correlated well with that of DSA.In the diagnosis of carotid artery stenosis, CTA has important strengthson the spatial measurement and orientation. It can be used as an preoperative diagnosis and evaluation method. In the cases with low degree of carotid artery stenosis, US has a higher sensitivity, accuracy and detected artio of plaque and can be used in screening and postoperative follow-up. In the patients who will not undergo interventional treatment, the complimentary application of CTA and US is expected to replace invasive examination of DSA.
