ObjectiveTo explore the diagnostic values of bile cytology during ERCP and blood tumor markers in malignant bile duct obstruction.MethodsTo estimate the diagnostic values of bile cytology during ERCP and blood tumor markers in malignant bile duct obstruction by the results of forty-seven cases whose bile duct obstruction between May 2008 to April 2009.Based on the results of operation, pathology and follow-up,the cases is fractionated to two groups. The malignant group has thirty-nine cases and the benign group has eight cases.ResultsThere are twenty-six cases positive in bile cytology test during ERCP,the sensitivity is 66.7 percents and twenty-eight positive in blood tumor markers, the sensitivity is 71.8 percents in the malignant group. There are eight cases negtive in bile cytology test during ERCP,the specificity is 100 percents and seven negtive in blood tumor markers, the specificity is 87.5 percents. The sensitivity of any one of bile cytology during ERCP and blood tumor markers positive is 89.7%.And the specificity of both positive is 100%.ConclusionsBile cytology testing during ERCP can provide the pathological evidence to diagnosis malignant bile duct obstruction and has clinic application value. The blood CA19-9 also is an available sign. The sensitivity is improved by combination test of bile cytology during ERCP and blood tumor markers. Especially it has important clinical application value for bile duct obstruction which is hard to identify malignancy or bening.
