ObjectiveTo evaluate the effectiveness and security of dispersing the liver and promoting the blood treatment on women with primary dysmenorrheal by randomized control study, and provide a more effective and practical method and scientific basis on it.Methods90 patients complied with both diagnostic criteria and dialectical standards of primary dysmenorrheal were chosen in the Clinic of Lin Jiaqing, Kaohsiung City from January 2009 to December 2009, which being divided randomly into Treatment group and Control group in average. The treatment group were treated by the method of dispersing the liver and promoting the blood, in which 5.5g Chaihu Shugan Soup and 7g Shaofu Zhuyu Soup(GMP herbal capsules) was taken by oral. The control group were treated with 12.5g Xiao Yao Wan(GMP herbal capsules). Both groups took 5 capsules for once,3 times a day with warm water, from 5 days before the menstrual to the second day of menstrual. The treatment was last for 3 menstrual cycles. Clinical symptoms,menstrual pain,quality of life (WHOQOL-BREF Scale),Self-Rating Depression Scale and safety index before and after treatment were observed and collected.The statistics were analyzed by SPSS 17.0. Segment information was inspected byχ2 test, rating information by Wilcoxon test, comparison of the means by t test or Wilcoxon test. The statistical graphics were made by Grapad Prism 4.03Results1. Clinical effect:the curative rate of the treatment group and the control group is 44.4% and 20.0%, while the total effectiveness is 91.2% and 80.0%. There are statistically significant differences in the two groups (P<0.05). The cure rate and the total effectiveness of the treatment group is higher than the control group, which shows that the method of dispersing the liver and promoting the blood can obviously release symptoms of dysmenorrheal, such as menstrual abdominal pain,less menstrual blood and anus bulge. The remission rate of the above is higher than the control group, which shows statistical significance (P<0.05)2. The comparison of the menstrual pain between the two groups after 3 months treatment shows statistical significance (P<0.05). The disappearance rate of the menstrual pain of the treatment group(71.1%) is higher than the control group(46.7%), which shows that the method of dispersing the liver and promoting the blood can lower the total score of the symptoms, and significantly relief dysmenorrheal.3. Effectiveness on syndromes of Chinese traditional Medicine:the curative rate of the treatment group and the control group is 24.4% and 11.1%, while the total effectiveness is 95.6% and 77.8%. There are statistically significant differences in the two groups (P<0.05). The cure rate and the total effectiveness of the treatment group is higher than the control group, which shows that the method of dispersing the liver and promoting the blood can predominantly improve syndromes of Chinese traditional Medicine.4. The patients feel obviously better after 3 months treatment in subjective sensation of living-quality and health,integration of physiology,psychology,social relationship and environment. It shows statistically significant differences (P<0.01). The treatment group has statistically predominance in patient's health,physiology and satisfaction for the environment than the control group (P<0.05). Both groups show therapeutic equivalence in feeling to living quality,psychology and sustaining to social relationship. The study shows that the method of dispersing the liver and promoting the blood can improve patient's living quality.5. Both groups lower down the SDS scores of patient with primary dysmenorrheal. The symptoms of SDS after the treatment is better relieved than the symptoms before the treatment. Comparing the two groups, the treatment group shows better effect.6. The safety detection:There is no abnormality seen of all the 90 patients before and after the treatment in hemanalysis,urinalysis,bowel analysis,hepatic function,renal function and ECG, which shows that the medicine is safe in clinical use.Conclusion1. The method of dispersing the liver and promoting the blood are more effective than the control group, which the comparison is statistical significant (P<0.05). And it can obviously alleviate the symptoms of qi-stagnancy and blood stasis,cold hygrosis.2. The treatment group has statistically predominance in patient's health,physiology and satisfaction for the environment than the control group (P<0.05). Both groups show therapeutic equivalence in feeling to living quality,psychology and sustaining to social relationship. The study shows that the method of dispersing the liver and promoting the blood can improve patients'living quality.3. Both groups can lower down the SDS scores of patient with primary dysmenorrheal.
