Research On The Relationship Between The Syndromes Of TCM And The Middle Molecular Substance In The Hemodialysis Patients

ObjectiveTo investigate the Syndromes of TCM and the relationship between the Syndromes of TCM and the middle molecular substance (MMS) in the hemodialysis patients。MethodsOn the basis of the existing diagnostic criteria of TCM syndrome, archival research, and clinical experience, undering the guidance of experts, to design the questionnaire of the four diagnostic information. According to the integration and elimination standard the samples were determined. Fasting blood samples that 2ml for a sample obtained before hemodialysis。Then fill in a form of TCM symptom survey.102 eligible samples were collected. Centrifugal blood evacuation experience, submission of samples not immediately were selected at-80℃temperature refrigeration. Qualified the four diagnostic data into different TCM syndromes by two physician-in-charge or above, excluding the views of non-uniform and the mixed symptoms had no significant tendency,96 patients eventually received. According to TCM syndrome all 96 patients were divided into 5 xu-groups:yin and yang deficiency, yang deficiency, qi and yin deficiency, qi deficiency, yin deficiengcy and a group else.Then according to TCM syndrome patients were divided into 6 shi-groups.ResultsThe symptom aspect, among the syndromes of deficiency in origin, Lassitude (67.7%), Weak waist (69.7%), palms and soles or aversion to cold, cold extremities (63.5%), thirst (69.7%), insomnia and dreaminess (70.8%) to present high-frequency; twitch and convulsions (72.9%) was the highest frequency among the syndromes of excess in superficiality. The dialectical aspect, for the asthenia syndrome group, deficiency syndrome of both YIN and YANG (27.7%) was the most,27 examples, next were Syndrome of spleen and kidney yang asthenia (25.3%),25 examples, what appears least was the Syndrome of kidney and liver yin asthenia (4.80%),5 examples, other were Syndrome of Qi and Yin deficiency (15.7%),15 examples, Syndrome of spleen and kidney qi deficiency (12.0%),12 examples. Among the syndromes of excess in superficiality, the stirring wind syndrome and the wind-dryness syndrome were most,69 and 76 examples respectively. Baseline information aspect, the asthenia syndrome group showed a certain trend with the mean age of hemodialysis:the Yin and Yang deficiency group> the Qi Yin deficiency group > the Yin deficiency group> the Yang deficiency Group> the Qi deficiency group) the group else. However, no significant differences between each group (P> 0.05). Only the Yin and Yang deficiency group had significant difference with other groups. The others had no significant difference between each other, there was a positive correlation between the concentration of MMS and the time of hemodialysis, r= 0.230, P<0.05. Compared with the mean value among the weakened body resistance groups:the Yin Deficiency group> the Yin and Yang Dificiency group> the Qi and Yin Dificiency group> the Yang Deficiency group > the Qi Deficiency group> the other group. Analysis of variance between groups was F=2.713, (P<0.05), there were significant differences in the mean value of MMS among the weakened body resistance groups. In pairwise comparison, there were significant differences between the Qi deficiency group and the Yin deficiency group (P=0.051), the Qi deficiency group and the Yin and Yang deficiency group (P=0.011), the Yin deficiency group and the group else (P = 0.043), the Qi and Yin deficiency group and the group else (P=0.047), the Yin and Yang deficiency group and the group else (P=0.008), the rest was not significant difference each other (P>0.05). There were no significant differences in the mean value of MMS among the shi-groups.ConclusionThe result showed that there was certain characteristics in Hemodialysis patients with different TCM syndrome, the syndrome of spleen and kidney yang asthenia and the deficiency syndrome of both YIN and YANG were increased significantly among the syndromes of deficiency in origin, and the stirring wind syndrome and the wind-dryness syndrome among the syndromes of excess in superficiality was much more. The distribution of MMS in different weakened body resistance group is significantly different, the Yin and Yang Dificiency group was the highest, lowest in the group else. It's showed that the Syndromes of TCM for the different levels of MMS may be an influencing factor.
