ObjectiveTo investigate the clinical effect of postoperative early enteral immunonutrition for the patients with gastric carcinoma who received total gastrectomy, through compare with postoperative early standard enteral nutrition and parenteral nutrition, then evaluate the applied value of the early enteral immunonutrition.MethodsEighty-seven patients with gastric carcinoma who received total gastrectomy during Mar 2007 to Oct 2009 in HangZhou First People's Hospital were randomly divided into 3 groups:immune-enhancing enteral nutrition group (IEN group, n=29), standard enteral nutrition group(SEN group, n=29) and parenteral nutrition group(PN group, n=29). Nutrient status and immune function of these three groups were detected on the postoperative day 1, day 7 and day 10. And the rate of postoperative infective complications were detected too.ResultsThe IEN group showed more effective than the SEN group and the PN group in improving postoperative nutrient status, immune function and nitrogen balance, whose indexs changed significantly on the postoperative day 7 and day 10 than that in the SEN group and the PN group(P<0.05). The rate of postoperative infective complications in the IEN group was 10.3%, lower than 34.5% in the SEN group and 37.5% in the PN group(P<0.05)ConclusionsPostoperative early enteral immunonutrition was safe and effective for the patients with gastric carcinoma who received total gastrectomy. Compared with the SEN group and the PN group, early enteral immunonutrition had significant effort in synthesizing protein, retrieving the immune function, lowing systemic inflammatory reaction, reducing the rate of postoperative complications.
