Background:The back and leg pain seriously disturbed people's lives for a long t ime,and lumbar disc herniation (LDH) is one of the factors causing low back pain, althou gh its diagnostic techniques, drugs and surgery have a significantly increased and improve d, but the LDH pain and disability to some extent has increased. Continued or severe pai n not only causes physical dysfunction, and severe depression, anxiety, fear and other psy cholog-ical disorder, caused by patients in the community, family not adaptation. Applicati on of evidence-based medical research to prove foreign cognitive behavior therapy (CBT) and e-xercise therapy, manipulation or drug therapy combined with a joint program can b ring th-e greatest clinical benefits, better than the application of a single method of treat ment. However, the medical profession due to inadequate attention, and the application of cognitive behavior therapy in Chinese medicine combined with conventional treatment stu dies have not been reported, it is necessary to develop a set of cognitive behavior therap y guided application of Chinese medicine applicable to cognitive behavior therapy in patie nts with LDH program.Objective:To explore a suitable TCM cognitive behavior therapy on the lumbar disc herniation patients, and in clinical practice, its feasibility, practicality and efficacy for examination.Methods:1. Through research the ancient and modern literature, to find principles of Chinese medicine on the the pathogenesis of LDH. present the latest progress of the treatment and Chinese medicine treatment of psychological cognitive behavioral characteristics.2. A TCM cognitive behavioral therapy intervention program on the basis of language enlighten, Suggestion therapy, Exposure therapy, Guide Qigong (Wu Qin Xi) therapy and combined with modern cognitive behavior therapy.3. parallel group randomized controlled clinical trial research methods,80 were diagnosed as LDH were divided into four experimental groups are compiled by the cognitive behavioral intervention program with Chinese Massage (A group), and lumbar fusion surgery (C) treatment; the control group respectively Massage (B) and lumbar fusion (D) treatment. To pain visual analogue score (VAS), pain behavior score (PBQ), SDS (SDS), self-rating anxiety scale (SAS) and the Oswestry functional disability index (ODI) and Prolo functional and economic lumbar disc herniation rating scale for the assessment tool results, respectively, before and after treatment and four patients were followed up for 6 months for the observation point in the score, the available information by independent sample t test and chi-square test and other methods of testing, from the pain behavior, bodily pain, depression, anxiety, dysfunction, such as economic status and treatment of patients with improvement compared.Results:1. Four groups Comparison of before and after treatment of pain behavior, bodily pain, depression, anxiety, dysfunction, economic status Indexes, the difference was statistically significant (P<0.05)2. After treatment Comparison of experimental group A and control group B, experimental group C and the control group D pain behavior, bodily pain, depression, anxiety, dysfunction, economic status Indexes of the experimental group than the control group to improve significantly, Statistically significant difference (P<0.05);3.6 months after the experimental C group and control group D Comparison of dysfunction and functional status of economic Indexes, the difference was statistically significant (P<0.05);4. Experimental group A and control group B Comparison of functional status and treatment of economic Indexes, Statistically significant difference (P<0.05);Conclusion:This study developed TCM cognitive behavioral therapy with Chinese Massage or lumbar fusion for the treatment LDH Significant synergism. Clinical intervention can be based on the actual situation of this treatment.
