Diagnostic Value Of 3.0T DCE-MRI, Apparent Diffusion Coefficient And Magnetic Resonance Spectroscopy In Breast Diseases

Objective: To approach the value of dynamic contrast-enhanced breast magnetic resonance scan (DCE-MRI) morphology, time-intensity curves, apparent diffusion coefficient (ADC value) and magnetic resonance spectroscopy(MRS) in the diagnosis breast diseases, to provide a richer, more valuable information for clinical preoperative diagnosis of breast diseases. Materials and Methods: Collected the doctor suspected breast lesions at our hospital needing further diagnosed 28 patients from July 2010 to March 2011 period, A total of 35 lesions,all are women,35 to 72-year-old, average 48-year-old. Preoperative magnetic resonance scan of conventional, diffusion-weighted imaging , dynamic contrast-enhanced scans and magnetic resonance spectroscopy, postoperative patients with pathologically confirmed, including 9 benign cases of 16 lesions, maximal diameter of lesions 0.8~7.1cm,average 3.2cm.19malignant cases of 19 lesions, maximal diameter of lesions 0.9~5.6cm, average 2.8cm .Analysis of breast DCE-MRI morphological performance (including the tumor shape and edge), DCE-MRI time-intensity curves, the measured ADC values and MRS in the identification of benign and malignant breast lesions.Results:①After dynamic contrast-enhanced,morphology of the sensitivity of diagnosis of breast cancer is 73.7%(14/19), specificity is 93.8%(15/16),accuracy is 82.9%(29/35);②Time-intensity curves of the sensitivity of diagnosis of breast cancer is 89.5%(17/19),specificity is 68.8%(11/16),accuracy is 80.0%(28/35);③Apparent diffusion coefficient (ADC value) of the sensitivity of diagnosis of breast cancer is 94.7%(18/19),specificity is 81.3%(13/16),accuracy is 88.6%(31/35);④M agnetic resonance spectroscopy(MRS) of diagnosis of breast cancer is 63.2%(12/19),specificity is 87.5%(14/16),accuracy is 74.3%(26/35);⑤Combining morphology and ADC values of the sensitivity of diagnosis of breast cancer is 98.6%,specificity is 76.2%,accuracy is 88.6%;⑥Combining TIC and MRS of the sensitivity of diagnosis of breast cancer is 56.6%,specificity is 96.1%,accuracy is 74.3%.Conclusion:①DCE-MRI of the breast in the differential diagnosis of benign and malignant disease,irregular shape,edge blur, blurred boundaries,filling-type ring enhancement,Ⅲ-type curve is typical of imaging findings of breast cancer;The shape is round or oval,sharp and smooth edges,homogeneous enhancement,Ⅰ-type curve of benign breast lesions diagnosis.②DWI with short time and without enhancement in breast diagnosis of benign and malignant lesions of the ADC threshold value was 1.222×10-3mm2/s,ADC < 1.222×10-3mm2/s was diagnosed malignant lesions,ADC≥1.222×10-3mm2/s was diagnosed benign lesions.ADC values of benign and malignant lesions of the sensitivity,specificity,and accuracy is high.It can be used as a routine examination sequence.③After combining morphology and ADC values,the sensitivity(98.6%) of diagnosis of breast cancer are improved.④After combining TIC curve and MRS,the specificity (96.1%) of diagnosis of breast cancer are significantly improved.
