Abstract Objective:Observation of atorvastatin calcium on curative effect of patients with aherosclerotic thrombotic cerebral infarction, as well as on serum lipid level and carotid artery intima-media thickness (IMT), high sensitive c-reactive protein (Hs-CRP), Interleukin 6 (IL-6), tumor necrosis factor-alpha (TNF-α), intercellular adhesion molecule-1 (ICAM-1) impact, thus discussion of atorvastatin calcium on cerebral infarction treatment effect and its mechanisms. Methods:The patients with cerebral arterial thrombosis were selected by the Diagnostic criteria for all stroke which was established by the Fouth Occasion Chinese Cerebrovascular disease academic conference and the results of CT or/and MRI; Who didn't accord with a int-reference or elimination standard were excluded.99 patients with cerebral infarction onset 48h were randomly divided into three group.Actual study done a total of 94 cases.General Group:32 cases, application of atorvastatin 10mg qd and aspirin 100mg qn; Intensive Therapy Groups:31 cases, application of atorvastatin 40mg qd and aspirin 100mg qn, Control Group:31cases, application of 100mg qn only. Respectively determination of the following indicators before treatment and after 24 weeks of the treatment:(1) Determination of NIHSS score and Barthel ADL index (BI) to research the effect of atorvastatin calcium; (2)Observation on adverse drug reactions to research safety of atorvastatin calcium in treatment of cerebral infarction; (3)Detection of blood lipid in patients with to research lipid effect of atorvastatin calcium; (4)Ddetection of carotid artery IMT, to research the atorvastatin calcium influence on carotid artery atherosclerotic plaque; (5)Detection of Hs-CRP, IL-6, TNF-alpha, ICAM-1, to research on anti-inflammatory effect of atorvastatin.Results: (1) Sets of clinical data, IMT, blood lipids and inflammatory indices of comparability among three groups:gender composition, average age, BI, NIHSS score score comparisons, no significant difference in three groups (P>0.05) before treatment; comparison of carotid artery IMT, no significant difference in three groups (P>0.05) before treatment; total cholesterol (TC), triglycerides (TG), low density lipoprotein-cholesterol (LDL-C), high density lipoprotein-cholesterol (HDL-C), no significant difference in three groups (P>0.05) before treatment; comparison of Hs-CRP, IL-6, TNF-alpha, ICAM-1, no significant difference in three groups (P>0.05) before treatment. (2) Effect of atorvastatin calcium on cerebral infarction:in general group and intensive therapy group, NIHSS score drop in value (7.90±5.34),(11.39±6.49) and the rising value of BI (37.10±7.81),(45.43±11.47) are better than in control group (4.66±4.47),(32.25±8.93)and intensive therapy group is superior to general group, statistically significant differences (P<0.05) after treatment. (3) Safety of atorvastatin calcium in treatment of cerebral infarction:in the three groups were not found during drug adverse drug reactions.(4)Effect of atorvastatin on serum lipid levels:TC, and TG, and LDL-Cdecline and HDL-C rose range respectively for (0.17±0.11, 0.20±0.12,0.09±0.06,0.01±0.05) in control group, TC, TG,LDL-C are have decline, and HDL-C has rose and in the treatment groups, range of intensive therapy group, respectively (1.93±0.16,1.29±0.17,2.13±0.09, 0.52±0.07) is greater than the range of general group (0.81±0.13, 0.68±0.15,1.36±0.07,0.59±0.06), statistically significant differences (P<0.05); range of intensive therapy group and general group are greater than the range of control group, statistically significant differences (P<0.05). (5)Effect of atorvastatin calcium on carotid artery IMT: Comparison of carotid IMT before and after treatment, general group without obvious changes, intensive therapy group has significantly reduced, control group has mild increase. Comparison carotid IMT drop among groups:intensive therapy group and general group drop stronger than the control group, intensive therapy group drop stronger than general group, differences are statistically significant (P<0.05).(6)Effect of atorvastatin on inflammatory media:Hs-CRP levels in all groups have decreased markedly after treatment:drop comparison among groups, intensive therapy group (9.76±5.41μg/L) significantly greater than general group (7.19±4.25μg/L), the latter is greater than control group (4.23±3.49μg/L), statistically significant differences (P<0.05). IL-6 levels in all groups have decreased markedly after treatment:drop comparison among groups, intensive therapy group (45.51±13.13ng/ L) significantly greater than general group (39.06±0.36ng/L), the latter is greater than control group (30.89±9.54 ng/L), statistically significant differences (P<0.05). TNF-αlevels in all groups have decreased markedly after treatment:drop comparison among groups, intensive therapy group (49.57±13.88μg/L) significantly greater than general group (39.75±12.11μg/L), the latter is greater than control group (29.20±9.37μg/L), statistically significant differences (P<0.05). ICAM-1 levels in all groups have decreased markedly after treatment: drop comparison among groups, intensive therapy group (299.32±69.56μg/L) significantly greater than general group (213.96±56.43μg/L), the latter is greater than control group (78.87±45.47μg/L), statistically significant differences (P<0.05).Conclusions:(1)Atorvastatin calcium has treatment effect on atherosclerotic thrombotic cerebral infarction; (2)Atorvastatin calcium on atherosclerotic thrombotic cerebral infarction treated with good clinical medication security;(3)Atherosclerotic plaque plays an important role in cerebral infarction, atorvastatin calcium is mainly through the stability of atherosclerotic plaque, control of atherosclerotic plaque formation to play a role in treatment in cerebral infarction; (4)Atorvastatin calcium mainly played effect of anti-atherosclerosis, stability of atherosclerotic plaque through the regulation of serum lipid and anti-inflammatory...
