Objective:To investigate effect of Ulinastatin on the expression of CD11b, ICAM-1, NE, ET-1 in pulmonary vein plasma level with cardiopulmonary bypass. Methods:40 Patients undergoing elective heart valve replacement were divided into into control group (group C, n=20) and ulinastatin group (group U, n=20), total of 1.2×104 U/kg ulinastatin were given intravenously before CPB. Blood samples were taken before CPB(T1),10 min after cross-clamping of aorta(T2),10 min after after bypass (T3), 1h after the termination of CPB (T4), for determination of Neutrophil CD11b were measured by FCM, serum ICAM-1, Neutrophils Elastase (NE) and Endothelin-1(ET-1) was measured by ELISA in vein plasma level. Result:1. there was no significantly difference between the two groups in the levels of CD11b, ICAM-1, NE, ET-1 at T1(P> 0.05).2. The levels of CD11b, NE, ET-1 increased significantly during CPB at different time. Compared to group C, CD11b, NE, ET-1 in group U were significantly lower after T1(P< 0.05).3. ICAM-1 at 1h after CBP(T4) in group U were significantly lower than that in group C (P< 0.05), then both groups increased compared after T1(P< 0.05).Conclusions:1. CPB is associated with release of proinflammatory cytokines and the expression of adhesion molecules in patients undergoing cardiopulmonary bypass, causing lung injury 2. Ulinastatin is effective in reducing this inflammatory response, protecting the function of important organs in the patients undergoing CPB.
