ObjectiveThe aim of this article is to explore the expression of CCR7 on non-small cell lung cancer and adjacent normal tissues,investigate the relationship between CCR7 expression and lymph node metastasis and clinical pathologic characteristic on non-small cell lung cancer, to provide reference for searching new target in treating malignant tumor and provide information for clinical individuation medication and prognosis judgment.Materials and MethodsForty cancer tissues and the adjacent normal tissues were collected from the patients with non-small cell lung cancer who underwent radical resection and lymph nodes dissetion in Taian central hospital from Octorber 2008 to April 2009, which were confirmed by pathology and the data was intact,All 80 tissues were examined by immunohistochemistry SABC method and RT-PCR method to detect the expression of CCR7 protein and mRNA,used SSPS 11.5 software package for statistical analysis, the difference of CCR7 expression according to the clinicopathologic factors was compared by x2 test, Logistic multivariate regressive analysis was performed to determine the independent risk factors of lymph node metastasis on non-small cell lung cancer.ResultsImmunohistochemistry method showed that in all 40 specimens,25(62.5%) lung cancer tissues and 6(15%)adjacent normal tissues were positive for CCR7, and there was a significant difference between them(χ2=19.01,P< 0.01),RT-PCR method showed that in all 40 specimens,29(72.5%) lung cancer tissues and 10(25%) adjacent normal tissues were positive for CCR7, and there was also a significant difference between them(χ2-18.06,P<0.01),With regard to lung cancer tissues, CCR7 expression was found higher in tumors with lymph node metastasis than in those without it (P<0.01),it was also related to differentiated degrees of lung cancer tissues(P<0.05),which meaned more CCR7 were detected in poorly differentiated lung cancer than in those highly,intermediately differentiated ones, it was found higher in tumors with T3-4 than T1-2(P<0.05),in PhaseⅢ-Ⅳthan PhaseⅠ-Ⅱ(P<0.05), this implied that CCR7 expression was significantly correlated with biological behaviour of non-small cell lung cancer. However, no significant correlations with gender, age, pathological types and general types were demonstrated for CCR7 expression. Immunohistochemistry method showed that the sensitivity,specificity,positive predictive value (PPV) and negative predictive value (NPV) of CCR7 expression for lymph node metastasis was 95.9%,77.8%,84*,93.3%. Logistic regression model analysis revealed that CCR7 positive expression in tumors(OR=6.798, P=0.0424)was independent risk factor of lymph node metastasis in non-small cell lung cancer.Conclusion1.CCR7 expression is high expression in non-small cell lung cancer tissues.2.CCR7 protein and CCR7mRNA expression has significant correlation to lymph node metastasis,. differentiated degree,clinical stage and T condition.3.CCR7 protein and CCR7mRNA expression has no significant correlation to gender, age. pathological types and general types.4. CCR7 positive expression could be a clinical index to foresee lymph node metastasis of non-small cell lung cancer.
