A Case Of Leg Ulcer Caused By Fusarium Moniliforme

Object A case of leg ulcer caused by Fusarium moniliforme is reported. To explore the clinical and laboratory features of leg ulcer caused by Fusarium moniliforme .Methods (1)The patient was taken the ulcer secretion to do microscopic examination and fungal culture repeatedly . The morphology of the fungus was observed by microscopy and scanning electron microscopy.(2)Taken the patient's skin lesion to do pathological examination.(3)To observe the curative effect with voriconazole and terbinafine in treating Fusariosis .Results (1)Direct smear:round and ellipse spore ,no hyphae。(2)Germiculture:no bacteria growth。(3) Fungal cultivation and identification :white and cottony mould colony grew out on PDA agars at the temperature of 25℃. Microculture: the separate , arborization and transparent hyphae were found, concatenate and capitulum-like microconidium. conidiophore is simple phialide. sickle-like macroconidia with 3～6 septa .The fungi could grow on Sabrous agar at the temperature of 25℃and37℃. Under scanning electron microscope: the microconidium like rosary , phialide and capitulum-like microconidium . It was identified as F. moniliforme. Chroic granuloma was found , but the fungal components were not observed by PAS and silver staining. Voriconazole and terbinafine are effective in treating Fusariosis. The lesions were cured by debridement and skingrafting of plastic surgery.Conclusions (1) The fungal form is sickle-like macroconidia, concatenate and capitulum-like microconidium. (2) Final diagnosis a case of leg ulcer caused by Fusarium moniliforme ,the combination voriconazole with surgery treatment showed significant curative effect in skin Fusarium infection.
