BackgroundArticular cartilage plays an important role in maintaining joint bearing and normal activity, cartilage disease are the important factor affect joint stability. Degenerative osteoarthritis (OA) is for clinical common diseases, OA is an important health concern, which leads elderly persons'disability of working and living, the pathogenesis is mainly articular cartilage degeneration and damage, it is often occous in the older people. Clinical used to diagnose OA mainly according to the cinical symptoms, physical examination and X-ray. These methods just provide the surface information of cartilage and it's thickness. These methods can't provide the information of cartilage degeneration at early stage. SO that it is difficult to diagnose OA at early stage, it was found when clinical symptoms appeared mostly. Arthroscopy is the gold standard for diagnosis of cartilage disease, but Arthroscopy is an invasive examination, it's higher technical requirements of the operator, for a low Vision, and exist the Observation of blind spots, can't provide the internal situation of the cartilage. Optical Microscope can observe the biochemical composition of cartilage, but need to cut organization, du biopsy, the process is very complicated, and it's an invasive. Magnetic resources imaging is noninvasive, have an advantage in the soft tissue resolution, Multi-parameter and Multi-planar imaging. Howerve, conventional MRI scans can only provide the cartilage thickness and shape. With the development of MRI technique, some MRI physiological imaging technical, such as T2 mapping, sodium imaging, delayed gadolinium-enhanced MRI of cartilage, diffusion weighted imaging(DWI) and magnetization transfer contrast (MTC) are expected to diagnose cartilage degeneration at early stage, which can be used as an evaluation and inspection tool for cartilage disease. T2-mapping reflects the constructure integrity of collagen fibre. These physiological imaging techniques are the focus of research.Objectives1. Getting the T2 values and T2-mapping of knee in superficial layer, the trasitional layer, the deep layer, respectively. And comparison between the three statistical differences. In order to provide the normal articular cartilage of biological tissue constitution change trend.2. Gettting the T2 values and T2-mapping of knee articular cartilage in diffent degree osteoarthritis patients and healthy volunteers, and comparison between the three statistical differences. To find the MRI T2-mapping in the knee OA constitutes the change of cartilage of biological tissue application value.MethodsBasis on the 2007 of Chinese Journal of Orthopaedics about OA treatment guidelines.117 cases of 102 accepting inspectors were divided into normal group (n=25) and OA group (n=77), and the OA group was categorized with the kellgren-Lawrence scoring system as mild OA group (n=39), moderate and severity OA group (n=38).They were examined with sagittal the 4-echo SE sequence. T2-weighted images were done and then were processed on GE ADW 4.4 using the FUNCTOOL software to construct T2-mapping. The T2 values in the cartilage of patella cartilage, the front of medial femoral cartilage, the edge of medial femoral cartilage, the front of lateral femoral cartilage, the edge of lateral femoral cartilage, medial tibial cartilage and lateral tibial cartilage were selected by drawing regions-of-interest (ROI)and calculated. And comparison the Statistically significant differences.Result1.The average T2 relaxation of normal group in superficial layer was (28.95±3.47) ms~(35.48±4.05)ms; the average T2 relaxation of transitional layer was (24.81±3.37) ms-(32.91±3.68) ms; the average T2 relaxation of deep layer was (22.67±3.27) ms~(30.50±3.54) ms; T2 in the superficial layer was statistical higher than in the transitional and deep cartilage (P<0.01). MR T2-mapping can reflect the trend.2. The Lvalues in the cartilage of patella cartilage, the front of medial femoral cartilage, the edge of medial femoral cartilage, the front of lateral femoral cartilage, the edge of lateral femoral cartilage, medial tibial cartilage and lateral tibial cartilage of normal group was (24.79+3.47) ms- (33.92±3.48)ms; the average T2, relaxation of mild OA group was (41.68±3.31) ms~(44.07±6.06) ms; the average T2 relaxation of moderate and severity OA group was (44.37±3.34) ms~(49.21±5.04) ms. The mean T2 value in the cartilage of osteoarthritis patients was higher than that of normal group. There was a statistically significant differences (P<0.01). There was a significent diffrence between mild OA group and moderate and severity OA group. MR T2-mapping can sensitively detect the biomechanics changes at the early stage of articular cartilage degeneration. It is a available method to diagnose the early degeneration, which has clinical directive value. The Tz-mappings show those changes of T2 values between the normal group and OA group.Conclusion1 The MRI T2-mapping technology can evaluation the knee cartilage each Layer of the organization structure1.1 The T2 values in the cartilage of patella cartilage, the front of medial femoral cartilage, the edge of medial femoral cartilage, the front of lateral femoral cartilage, the edge of lateral femoral cartilage, medial tibial cartilage and lateral tibial cartilage of the superficial layer was statistical higher than in the transitional and deep cartilage, they were in turn decrease.1.2 The T2 values in the cartilage of patella cartilage, the front of medial femoral cartilage, the edge of medial femoral cartilage, the front of lateral femoral cartilage, the edge of lateral femoral cartilage, medial tibial cartilage and lateral tibial cartilage of mild OA group was statistical higher than normal group.1.3 The T2 values in the cartilage of patella cartilage, the front of medial femoral cartilage, the edge of medial femoral cartilage, the front of lateral femoral cartilage, the edge of lateral femoral cartilage, medial tibial cartilage and lateral tibial cartilage of moderate and severity OA group was statistical higher than normal group, and was statistical higher than mild OA group.2 The T.-mapping can evaluation the knee cartilage each layer of the organization structureMR T2-mapping can reflect the change of biochemical content in the cartilage of knee joint. It also can show the component abnormality with higher T2 values in mild OA without morphological change. MR T2-mapping has an important value in the diagnosis of mild OA.
