The policy of reform and opening up has been taken for 30 years, China's economy has been developing rapidly, and the gap between rich and poor is made larger, the form of society has been changed tremendously, these changes affect the Chinese people through all aspects of their life, including the Medicare security systems which has undergone several changes. From the beginning of the founding of the PRC, we use the rural medical form called "Cooperative medical service, the bare foot doctor, Counties, towns and villages the three levels of health care networks". And now, we are using the new rural cooperative medical service. In the process how to maintain fairness has been questioned all the time.The paper studies the low income farmers who joined the new rural cooperative medical service. The research contains five segments:The first part includes the purpose and significance about this research, and the structure of the paper. And it describes the recent situation of the new rural cooperative medical service.The second part defines several concepts, and introduces the related theories which including the theory of justice, the theory of new welfare economics and the model of Horn and Meter.The third part introduces the status quo of the new rural cooperative medical service. And empirical point of view, choose Jingzhou as the investigations' area. Use questionnaire and interview to do the research.The forth part analyzed the data based on the questionnaire, and describes the reasons why there is unfairness. According to the system theory, we look for the reasons from the aspect of the environments and the system itself.The last part of the paper proposes some corresponding countermeasures. First is the system of improving the environment and second is to optimize the system itself.
