[Objective] To evaluate emergency operation and transcatheter arterial embolization (TAE) for the hemorrhage of ruptued hepatocellular carcinoma for a spontaneous rupture of HCC.[Methods] According to the therapeutic programs 90 patients with bleeding HCC were divided into two groups:group A (n=50), receiving transcatheter arterial embolization. (TAE); group B(n= 40), receiving emergecy operation. The hemorrhage of ruptued hepatocellular carcinoma were evaluated bye the successful rate of hemostasis, The liver function resume, The in-hospital mortality rate, The abdominal cavity metastasis rate, the one-year survival rate, and the two-year survival rate.[Results]:The successful rate of hemostasis in the two groups were 96% and 100%, but the difference showed no statistically significance (χ2=1.636, P= 0.201>0.05), The liver function resume in (9.2±3.4days) in groupA, and (13.5±4.7 days) in goupB, the difference was proved to be significant (P<0.05). The in-hospital mortality rate in goupA(10%) was significantly lower than those in goupB(27.5%), the difference showed statistically significance (p=0.013<0.05)。The radical cure rate of operation follow TAE was (100%) and (67.2%) in goup B, the difference showed statistically significance (P=0.002<0.05).The abdominal cavity metastasis rate were (17.4%) and(5%), showed no statistically significance (P=0.107>0.05).Theone-year survival rate (60%)and (75.8%), (p=0.159>0.05) two-year survival rate(42.2%)and (51.7%) (p=0.276>0.05), the difference showed no statistically significance.[Conclusions](1)TAE has become an effective way in controlling bleeding from ruptured HCC in acute phase. (2) TAE used for the bleeding of ruptured HCC is helpful for raise the radical cure rate in two-stage operation.
