Objective: To investigate the effects of different fluids (sodium lactate Ringer's solution and sodium pyruvate Ringer's solution) on the survival rates, visceral functions and metabolic acidosis during the resusciation of lethal hemorrhagic shock in rats.Methods: 1.Chose male Sprague-Dawley rats,with the body weight between 280g and 300g.Fixed rats on the operating table with supine position after they were anesthesised with ketamine and Sumianxin II.Disinfected local skin with Iodine,then took on the cannula in ipsilateral of thigh arteriovenous with PE-50 catheters,and in ipsilateral of carotid artery,which was connected with Picco cardiac function monitor for mean arterial pressure (MAP),while maintained ventalation and normal temprature.Rats were heparinized through the femoral vein,then drawn 45% of Total blood volume(TBV)body blood volume through femoral artery catheter within 20min (TBV= body weight×0.06+0.77[1]).MAP was maintained within 35-40mmHg for one hour. Hemorrhagic shock was made. 2.After drawn the blood,Chose rats which were still alive after one hour among 60 male SD rats, randomly divided into(1)sodium lactate Ringer's solution group(LR),(2)sodium pyruvate Ringer's solution group (PR), (3)Pure blood loss(Control).After the liquid were warmed to 37℃,rats were infused the liquid with the volume 3 times of the blood loss through intravenous catheters with trace pump uniformly within one hour.Removed the catheter one hour after the infusion finished and ligated the vessle, sutured the skin,and put the rats back in the breeding cage,kept warm,and offered foods.Observed the survival rate of 1h, 4h and 24h after resuscitation for survival analysis.Among the other 40 rats,same process were repeated,randomly divided into(1)Sodium lactate Ringer's solution group(LR), (2)sodium pyruvate Ringer's solution group(PR).All rats were drawn 0.5ml of blood though femoral artery,when prehemorrhage,1h after hemorrhage,1h after resuscitation and 4h after resuscitation.Heart, liver, spleen, lung, kidney, jejunum and brain were taken out 4h after infusion.3 Some of the blood were used to test arterial blood gas.Blood plasma were taken to test blood biochemical and lactic acid.The organs were used to measure the moisture of visceral organs.Results: 1.Analysis of MAP and survival rate: MAP rapidly falled to bellow 30mmHg,maintained 35-40mmHg during shock stage.MAP rose gradually following the fluid resuscitation,then Picked up to 55-60mmHg about 20min after resuscitation,and up to 70-75mmHg at the end the resuscitation.There were no obvious difference between the two groups. During 1h to 2h after resuscitation,MAP has slight decrease in PR group, but were still up to 65mmHg;MAP has obviously decrease in LR group,about 40-45mmHg.There were 50 rats alive 1h after hemorrhage.Survival rate 1h after resuscitation: Control group 44.4%(8/18),PR group 85% (14/16), LR group 62.5% (10/16).Survival rate 4h after resuscitation: Control group 27.7%(5/18),PR group 75%(12/16),LR group was 50%(8/16).Survival rate 24h after resuscitation: Control group5.5%(1/18),PR 50% (10/16), LR group for 18.7%(5/16).2.Blood biochemical (CK-MB,Cr,ALT )levels rised obviously after hemorrhage.There were no difference between groups. After resuscitation, there were obviously improved in PR group(P<0.05).3. Arterial blood gas and Lactic acid: there were severe acidosis after blood loss,but no difference between groups.1h and 4h After resuscitation, PR group were superior to the indicators to LR group (P<0.05). 4. Water content of viscera 4h after resuscitation: PR group were significantly lower than LR group.Conclusion: Comparing with sodium lactate Ringer's solution, administration of sodium pyruvate Ringer's solution on the resusciation of hemorrhagic shock in rats can obviously correct acidosis,reduce viscera edema, thereby improve viscera function and the survival rate.
