Background and objectiveThe recurrence and metastasis is one of the main reason for the treatment failure of Hepatocellular carcinoma, even small liver cancer underwent radical resection, the 5 years postoperative recurrence rate was up to 40%~50%, and the large liver cancer 5 years recurrence rate after operation was as high as 80%,about 66.7%~77.2% patients's cause of death was associated with recurrence and metastasis of hepatocellular carcinoma and the re-resection rate of the hepatocelluar carcinoma after its recurrence was less than 30%.Lymphatic metastasis is a one channel of metastasis of hepatocelluar carcinoma, autopsy confirmed lymphatic metastasis rate of hepatocellular carcinoma was about 25.5% ~ 32.9%. The literature reported that 5 years after postoperative recurrence rate of hepatocelluar carcinoma with portal hepatic lymophatic metastasis was 82.05%.CXCR7 and CCR7 were discovered for few years,researchers found that the two chemokine receptors were involved in lymphatic metastasis of tumors , so we detect the expression of CCR7 and CXCR7 protein of the hepatocellular carcinoma, to analyze the relationship between the expression of CCR7 and the CXCR7 and the lymphatic metastasis of hepatocellular carcinoma, and then to investigate the influencing factor of lymphatic metastasis and seek a new way f or the treatment of the cancer.MethodsThe tissue samples including hepatocellular carcinoma,the adjacent tumor tissues (0.3~0.5 cm from the edge of the tumor) , the far tumor tissues (5 cm from the edge of the tumor), were obtained from 48 patients with clinical data and consent Integrity. The expression of CCR7 and CXCR7 by immunohistochemistry whith metastasis of porta hepatis lymph nodes in hepatocellular carcinoma.Results1. In the immunohistochemical staining we found the CCR7 protein mainly located in the cell envelopes and/or cytolymphs. The positive rates of CCR7 protein in hepatocellular carcinoma,in adjacent tumor tissues and in far tumor tissues were respectively75.00%(36/48),62.50%(30/48) and27.08%(13/48) .2. There are 16 case, the positive of CCR7 protein in tumor tissues with portal hepatic lymoph atic metastasis were 87.50%(14/16),however the positive of CCR7 protein in tumor tissues with no portal hepatic lymophatic metastasis were 53.13%(17/32), in 48 hepatocellular carcinoma Patients. Comparing the two groups , there were also significant differences P<0.05.3. In the immunohistochemical staining we found the CXCR7 protein mainly located in the cell envelopes and/or cytolymphs. The positive rates of CXCR7 protein in hepatocellular carcinoma,in adjacent tumor tissues and in far tumor tissues were respectively87.50%(42/48),64.58(31/48)and27.08%(13/48) .4. There are 16 case, the positive of CXCR7 protein in tumor tissues with portal hepatic lymoph atic metastasis were 100.00%(16/16),however the positive of CCR7 protein in tumor tissues with no portal hepatic lymophatic metastasis were 68.75%(22/32), in 48 hepatocellular carcinoma Patients. Comparing the two groups , there were also significant differences P<0.05.Conclusion1.The expression of CCR7 in hepatocellular carcinoma,the adjacent tumor tissues was high, however, it was low in the far tumor tissues.2.The expression of CCR7 in the tumor tissues with porta hepatic lymph nodes was higher than with no porta hepatic lymph nodes. it minght indicate that CCR7 was closely with the lymphatic metatasis of the hepatocellular carcinoma.3. The expression of CXCR7 in hepatocellular carcinoma tissues was high and was middle level in the adjacent tumor, however, it was low in the far tumor tissues.4.The expression of CXCR7 in the tumor tissues with porta hepatic lymph nodes was higher than with no porta hepatic lymph nodes. it minght indicate that CXCR7 was closely with the lymphatic metatasis of the hepatocellular carcinoma.5.The high expression of CCR7 protein and CXCR7 protein in hepatocellular was closely related to the lymphatic metatasis and recurrence.
