Objective:To observe the effectiveness and safety of HIFU(High intensity focused ultrasound) in hypersplenism therapy.Methods:19 patients randomly selected of cirrhosis or liver cancer with hypersplenism were treated by HIFU ablation of the spleen,observe the exchange of red blood cells, white blood cells, platelets before and 1 week , 1 month, 3 months, 6 months after HIFU treatment;observe the exchange of liver function before and 1 week , 1 month after treatment;also the splenic volume changes before and 1 month after treatment ;and complications occurrence as abdominal pain, fever,chest and abdominal cavity effusion .Results: After 1 week, 1 month, 3 months, 6 months leukocytes was significantly higher than preoperative (P<0.05); after 1 week, 1 month, 3 months platelets were significantly higher than that before operation (P <0.05 ), but 6 months postoperatively no significant increase compared with the preoperative platelet (P > 0.05); after 1 week, 1 month, 3 months, 6 months no significant increase compared with the preoperative red blood cells (P > 0.05); after 1 week, 1 month liver function was significantly improved than preoperative ;splenic volume after 1 month was significantly reduced compared with the preoperative (P > 0.05). Left upper abdominal pain occurred in 3 cases, fever in 4 cases, left pleural effusion in 1 case, 1 patient after symptomatic treatment of abdominal pain lasted about 1 month, the other complications all disappeared within 1 week, does not appear ruptured spleen, splenic abscess, perforation of hollow organs and death cases.Conclusion:Clinical use of HIFU treatment of the spleen is effective and safe , and liver function can been improvede ,but the correlation of the efficacy, safety and ablation volume, needs further exploration.
