| Objective:To explore the status and influencing factors of Hand-Foot-Mouth Disease from 2008 to 2009 in Changsha, in order to provide scientific basis for Hand- Foot-Mouth Disease prevention.Methods:With cross sectional study, the epidemic situation report data of Hand- Foot-Mouth Disease from 2008 to 2009 in Changsha were collected. Case survey were made among all of the Hand- Foot-Mouth Disease cases in Hunan Children's Hospital and Hunan People's Hospital in 2008~2009. And the suspected case of Hand- Foot-Mouth Disease from 2008 to 2009 in Changsha were investigated and detected by the lab. Epidemic report and case survey were analyzed by SPSS 13.0. Count data was described with ratio and composition ratio. Influencing factors of HFMD was described with Multiple linear regression and logistic regression. Etiology examination was implemented with RT-PCR.χ2 test and logistic regression were used to analyzed diagnosis cases and negative cases in virus test.Results:During 2008-2009,13613 HFMD cases were reported in Changsha, with an annual average incidence rate at 110.87/100,000. Region distribution:All the five areas of Changsha reported HFMD cases, and Yuhua area had the highest incidence. Season distribution: Cases occurred throughout the year with two peaks—April to June and December. Population Distribution:Many cases were scattered children (68%).The incidence of children aged 1-4 was highest. The incidence of male was 140.72/100,000(female:79.30/100.000).48.6% HFMD cases had fever, and 77.4% had rash. Multivariate analysis showed that younger (b'=-0.127), fever (b=0.152) treatment delay (b'=0.348) were the risk factors of extended course of disease. Logistic Regression Analysis showed that kindergarten support (OR=2.393,95%CI:1.136~5.041), contact history (OR=3.836,95%CI:1.436~10.250), Finger (toy)sucking (OR=4.213,95% CI:1.395~12.721) were related with positive in virus test.Conclusion:The distribution of HFMD has region and gender disparities. Many cases were children aged 1-4.The prevalence peak is during April-June. The course of HFMD is 5 days. The main symptoms are fever and rash. Younger, fever and treatment delay are the risk factors of extended course of disease. Contact history,Finger (toy) sucking and kindergarten support are the independent factors of positive in enterovirus test, and they may be one of determinates factors of HFMD infection. |
