Objective The objective of this paper is to analyze the epidemiological characteristics of HFMD in Hefei City and identify the primary risk factors of Hand-foot-mouth disease of children under five years old in Hefei rural area and to provide the scientific data for the prevention and control of HFMD.Methods Collect and analyze the data of all patients with HFMD in Hefei City during 2008-2010. Laboratory testing was performed on all severe cases, all agminated cases and part of general cases. Information about the general characteristics, family environment and life style of 83 cases and 83 controls was obtained during the telephone interview.Results 16455 cases were reported in the last three years, while 220 were severe cases and 6 were died. An upward trend was found on morbidity of HFMD year by year. Male and female were in the ratio of 1.77:1. About 88.5% cases were younger than 5 years old and 96.9% were nursery and kindergarten children or live scattered kids. The reason peak appeared from April to June. HFMD in Hefei City were mainly caused by human enterovirus 71 (HEV71), positive rate was 54.4%. Multiple logistic regression model showed that scattered inhabiting children(OR=1.99, 95%CI:1.26-19.75), lower education of guardian(OR=3.54, 95%CI: 1.14-10.96), skin eruption(OR=234.93, 95%CI: 15.34-3598.11), history of contact with patients (OR=4.27, 95%CI: 1.92-14.81), no habit of washing hands before meals or after using the restroom(OR=3.84, 95%CI: 1.14-16.66), faces piling up around house environment(OR=20.92, 95%CI: 2.18-200.04) were the main risk factors of Hand-foot-mouth disease of rural children under five years old.Conclusions HFMD in Hefei has unique epidemiological characteristics, Poor living environment and habits were main risk factors for Hand-foot-mouth disease of rural children under five years old. comprehensive measures should be taken to prevent and control the epidemic of HFMD.
