With the rapid development of world economy,and people's living standards improve, diabetes mellitus has become a new epidemic diseases.Now there are about 200 million diabetes patients all the world,the member will expected to reach 300 million by 2025.Additional patients with diabetes mellitus about 3/4 are in the developing countries including China. The latest epidemic ological survey of diabetes in China show, about 1 million people with diabetes in adults over the age of 20,account 9.7%.The number of pre-diabetes or impaired glucose tolerance are about 15.5%.Growing very fast.Facts proved that China has surpassed India as the superpower of diabetes. The Diabetic retinopathy is the most common microvascular complications of diabetes. The Diabetic retinopathy occurred in patients of diabetes in China was 25%, of which newly diagnosed patients with type 2 diabetes,the incidence rate of diabetic retinopathy as high as 12%. Severity of diabetes associated with course of disease and Degree of Metabolic disorders and so on. Course of diabetes are more than 10 years, more than 50% of patients associated with different degrees of diabetic retinopathy. Currently diabetic retinopathy has become one of the major blinding diseases.Diabetic retinopathy are mainly based on retinal vascular lesions, its pathogenesis is very complex, including several aspect such as: Accumulation of advanced glycosylation end products,Increased polyol pathway,Activation of protein kinase C,Oxidative stress and Over-expression of cytokines etc.With the development of molecular biology,pathogenesis of DR Study on the molecular level is more and more attention. In recent years, many studies have found that many cytokines plays an important role in the development and progression of DR. VEGF is one of the strongest endothelial cell mitogen and cytokines induced angiogenesis. VEGF is now considered most closely associated angiogenesis factor as one of Cytokines.IGF-I as a multifunctional cell proliferation regulatory factors is closely related with growth and reproduction of a variety of retinal cells.In order to investigate VEGF and IGF-1 of cytokines in the pathogenesis of diabetic retinopathy, we select 60 patients hosp- italed and outpatients in Japan Friendship Hospital department of Endocriology from March 2009 to mid-2011,which diagnosed as type 2 diabetes .patients were divided into type 2 diabetes is not associated with diabetic retinopathy group (NRD) 20 cases, type 2 diabetes with background diabetic retinopathy group (BDR) 20 patients, with proliferative diabetic retinopathy group (PDR) 20 patients and healthy control group (Normal) 20 patients. All patients were examined body mass index (BMI), fasting plasma glucose, glycosylated hemoglobin, triglycerides, total cholesterol, low-density lipoprotein cholesterol and VEGF, IGF-1 levels in serum ,then compare all of data.The measured data applications,applications spss13.0 statistical software for data processing. P<0.05 stand for data with statistically significant, P <0.01 represent serious statistically significant .Though statistical and analysis of clinical data, we found(1) Compared all the group of sex, age and BMI ,data show there are not statistically significant; course of desease: the BDR group compared with the NDR group, the diffirence is statistically significant (p <0.05); the PDR group>the BDR group> the NDR group 0.05).(3) VEGF levels: VEGF levels in type 2 diabetes groups compared with the level of the control group were significantly increased (P <0. 005 ~ 0. 05), the difference was statistically significant; BDR group compared with the NDR group, VEGF level was significantly higher than the NDR group, PDR group compared with the BDR group,VEGF levels decreased, but still slightly higher than the NDR group.(4) IGF-1 levels: IGF-1 level in type 2 diabetes groups compared with the normal control group were significantly higher (P <0. 005 ~ 0. 05), difference is statistically significant; addition, NDR group compared with the control group P <0. 01, BDR group compared with the NDR group P <0. 05, PDR group compared with the BDR group P <0. 05, showing NDR group < BDRgroup 0. 05).This study shows that :(1) Type 2 diabetic patients with diabetic retinopathy was positively correlated with the duration of disease, with the extension of duration of diabetes, diabetic retinopathy of severity progressive increasing. VEGF levels in the NDR and BDR groups showed an increasing trend.But the PDR group's VEGF levels decreased than BDR,still slightly higher than the NDR group. VEGF is closely related to the period of active neovascularization in diabetic patients,which including NDR and BDR. VEGF participate in the development of diabetic retinopathy, can be used as one of the monitoring indicator of the severity of diabetic retinopathy.(3) With the severity of diabetic retinopathy, IGF-1 levels gradually increased, IGF-1 participate in the development of diabetic retinopathy, can be used as one of the monitoring indicator of the severity of diabetic retinopathy.
