Research backgroundPrimary nephrotic syndrome is a clinical syndrome that primarily in the glomerular with macroalbuminuria,hypoproteinemia, hyperlipemia and edema. Glucocorticoid is the well-recognized main drug treatment for PNS. Although it shows good curative effects, it also caused problems such as the relatively long course of treatment. significant toxicity, steroid dependence, steroid resistance, or frequent relapses.In recent years, many scholars study on using traditional Chinese medicine to treat PNS, their researches suggest that combined therapy of traditional Chinese and Western medicine has good curative effects, alleviating the side effects caused by glucocorticoid, reducing recurrence rate, enhancing the quality of life, so this method is worth further study and research.Research objectsXinjiachunze Decoction is a proved prescription created by well-know TCM physician—Professor Gao Huiyuan, he and my advisor use this decoction for decades, proved it's significant effect on treating nephrotic syndrome. sum up my advisor's experience, to observe the curative effect of xinjiachunze decoction combined with glucocorticoid, evaluate the feasibility and research value of this method, discuss the mechanism of Xinjiachunze Decoction, enrich the thoughts for the treatment of combination of TCM and Western Medicine.Research methodAccording with the diagnostic standard of primary nephrotic syndrome and deficiency of spleen-yang and kidney-yang,42 patients are divided into treatment group(21cases) and control group(21cases). The control group takes Prednisone 1mg/kg·d orally for eight weeks. The treatment group adds with Xinjiachunze Decoction on the base of control group. All the patients has been given the same conventional treatment at baseline. The treatment course is eight weeks, observe the changes of TCM syndrome scoring, therapeutic indexes and laboratory indexes, analyze and discuss the results with statistics. Research results1. Clinical effects:the overall response rate of treatment group is 85.7%, much higher than that of control group-71.4%. (p< 0.05)2. TCM symptoms effects:both two groups can availably improve the TCM symptoms (p< 0.05), treatment group has a more significant effect on improving those TCM symptoms:edema,intolerance of cold,chills,loose stool (p< 0.05).3. The treatment group has better clinical efficacy than control group on decreasing the urine protein, increasing the serum albumin, regulating blood lipids(p< 0.05).Both two groups can improve the blood high congealment condition (p< 0.05), and have no effect on renal function (p> 0.05)Research conclusions1. Xinjiachunze Decoction has significant effect on treating PNS (deficiency of spleen-yang and kidney-yang),it can decreasing the urine protein, promote the level of serum albumin, improve the lipid metabolism disorder and TCM symptoms like lumbago,edema,intolerance of cold,chills,loose stool with no significant adverse side effects.2. Besides the conventional glucocorticoid treatments, the combination of Xinjiachunze Decoction give more favorable results for PNS (deficiency of spleen-yang and kidney-yang), the clinic method is worth to be studied further.3. Tonifying spleen and kidney, activating qi and excreting water could be the treatment principle for PNS (deficiency of spleen-yang and kidney-yang), modification according to symptoms, satisfied effect will achieve.
