The objective of this article is to investigate the treatment mechanism of the Chinese medicine of tonifying the kidney for postmenopausal osteoporosis (PMOP).The first part is a review of investigation of Traditional Chinese Medicine for PMOP. Many physicians considered the most important pathogenesis of PMOP is deficiency of kidney essence. Experimental study found that the Chinese medicine of tonifying the kidney by adjusting and related molecules of bone cells to improve bone metabolism markers, improve bone mineral density, so as to treat disease. In addition, Chinese medicine in preventing the disease-related research has also made some progress.The second part is the review of modern medicine. It explains the aspects of modern medicine for PMOP from the epidemiology, diagnosis, laboratory examinations (biochemical markers of bone metabolism), prevention and treatment.The third part is a clinical study:Objective:To observe the treatment mechanism of the Chinese medicine of tonifying the kidney for PMOP.Subjects and Methods:A total of 134 cases of PMOP patients completed the study. Patients were randomly divided into active vitamin D+calcium group (control group) and the Chinese medicine of tonifying the kidney+Ca+active vitamin D group (Chinese medicine group), and two groups treated for 1 year. At 0 month,3months,6 months all patients were tested urinary calcium/creatinine (Ca/Cr), urinary type I collagen cross-linked C telopeptide/creatinine (CTX-I/Cr).And at 0 month,12months they were examined the bone mineral density.Results:All patients of urinary Ca/Cr were not significantly different. But the Chinese medicine group's urinary CTX-I/Cr decreased (P<0.01) at 3 months, and femoral neck BMD increased significantly (P<0.05). Urinary CTX-I/Cr changes was positively correlated with the changes of femoral neck BMD (P<0.05). The control group urinary CTX-I/Cr, femoral neck/lumbar spine BMD were not significant statistically.Conclusion:The Chinese medicine of tonifying the kidney can fill the kidney essence, nourish the liver and the kidney functions, decrease bone resorption, and at the same while enhance bone formation, increase bone mineral density. The mechanism may be related to inhibit the resorption of osteoclast and promote apoptosis of osteoclast, and promote the differentiation and proliferation of osteoblast to promote bone formation.
