Clinical Research On Method Of Jian Pi Shu Gan On The Effect Of Blood Glucose Levels In Patients With Pre-diabetes

BackgroundDiabetes mellitus (DM) is a serious health problem that many countries are facing, IGT and IFG are special metabolism conditions that between DM and normal blood glucose,which are all called IGR or can benamed as Pre-diabetes, they indicate that the risk of individuals occurring DM and CVD even early death are increasing in the future. The American diabetes association (ADA) diagnostic criteria for IGR is fasting blood glucose≥5.6mmol/L-<7. 0mmol/L, or 2 hours after postprandial bloodglucose≥7.8mmol/L-<11.1mmol/L. In 1996, national survey of the prealence of diabetes and IGT were 3.21% and 4.76%, according to the latest survey, China's current of prealence of diabetes and pre-diabetes is 9.7% and 15.5%, which is a clear upward trend. The treatment costs about Diabetes and ensuing complications have brought heavy burden to society,family and individuals. How to taking interventions to stop the development of diabetes and the occurrence is becoming the core of many problems that medical workers are discussing.The clinical manifestations of pre-diabetes and diabetes are similar, typical symptoms are mouth parched and tongue scorched,hungry for food & frequency of urinatior,fatigue & thin etc, but there are also some patients who don't have such symptoms, whose abnormal blood glucose can be found after a health check. According to the typical clinical manifestations, such symptoms are attributable to traditional Chinese medicine "Diabetes" category, ancient physicians have a unique insight on its etiology and treatment. Quoted the diabetes discussions of ancient physicians and glucose metabolism clinical research in patients with pre-diabetes by modern medical scientists, we find that special spleen and liver-depression play important roles in the pathogenesis of IGT or IFG, what's more, Chinese medicine has a unique advantage in its treatment.ObjectiveThe jian pi xiao tang particles which is made according to the method of jian pi shu gan can stopping the development of diabetes and nipping it in the embryonic stage. At the same time, safety is evaluated after observing the clinical effect on blood levels of pre-diabetes patients who are treated with the method of jian pi shu gan.MethodIGR patients who meet the diagnostic criteria or are involved randomly divided into three groups, which are life intervention group (intervention group),jian pi xiao tang particles & metformin group (Chinese & Western medicine group),metformin alone group (Western medicine group), those three groups are all given life interference. FPG,2hPG,HbAlc,lipid,FINS,Fasting c-peptide,liver,kidney function etc are detected for each patient before and after treatment. There are three courses (one course treatment lasts for a moth), and then we find the changes of relevant indicators and discuss Clinical syndrome and Possible factors in IGR patients, at last evaluate the efficacy and safety of jian pi xiao tang particles that affected on lowing IGR patients'blood glucose levels.The results90 IGR patients were included in this study, and there were 3 groups whose fasting blood glucose and postprandial blood glucose levels were decreased compared before treatment with after treatment, the Chinese & Western medicine group decreased significantly. After comparison of 3 groups of patients before and after treatment, TG,TC, LDL-C level did not decrease significantly in intervention group and Western medicine group While Chinese & Western medicine group reduced more significantly. After comparison of HbAlc,FINS,Fasting c-peptide before and after treatment, intervention group did not change, HbAlc and FINS,Fasting c-peptide were improved in Chinese & Western medicine group and Western medicine group, but Chinese & Western medicine group more significantly.ConclusionThis study found that IGR patients blood glucose levels can be decreased through life intervention, taking jian pi xiao tang particles and metformin, while more effective treatment for IGR can be got through the combination of traditional Chinese medicine jian pi xiao tang particles with metformin, and improve the HbAlc and FINS,Fasting c-peptide,lipid.
