Objective:To observe the serum level of inflammatory factors in pig extremities with multiple gunshot wounds after treatment with vacuum sealing drainage(VSD). To study the early effect of vacuum sealing drainage (VSD) with instillation on the infected gunshot wounds.Methods and results:have two part:Part one:Debridement of gunshot wounds in extremities of 12 pigs was performed 6 h after injury. The pigs were randomly divided into treatment group and experiment group. Pigs in treatment group received traditional treatment, while pigs in experiment group was treated with VSD. Serum C-reactive protein (CRP) and IL-6 levels were measured in the two groups by ELISA, we dealt the resuts with statistical method. Shows that no significant difference was observed in serum CRP and IL-6 levels before and 6 h after injury between the twp groups(P>0.05). The serum CRP and IL-6 levels increased gradually in the two groups and reached their peak at 48 and 24 h respectively after injury. However, the serum CRP and IL-6 level was significantly lower in experiment group were than in treatment group(P<0.01).Part two:18 pigs were shot in right hind, all the wounds were left untreated until the third day after injury when they were infected. Pigs were divided randomly into control group, VSD group and experimental group (n=6/group). The control group was treated with conventional therapy, experimental group was treated with VSD with instillation. Bacterial counts of wound tract, the level of leukocytes and endotoxin in peripheral blood of three groups were detected at different time after different treatment, and observed conventional pathology. shows that compared with the control group and VSD group, bacterial counts of wound tract decreased sharply(P<0.01), the level of leukocytes and endotoxin in peripheral blood drop rapidly(P<0.01), the results of pathology showed that inflammatory response reduced, tissue repair accelerated.Conclusions:VSD can significantly decrease the serum level of inflammatory factors in pig extremities with multiple gunshot wounds and systemic inflammatory response. Use VSD with instillation in treatment of infected gunshot wound, it can rapidly control infection, reduce the systemic response, create a good soft tissue conditions for repair and reconstruction as early as possible.
