Purpose:Oral squamous cell carcinoma ( OSCC ) is the most common type in oral-maxillofacial malignant tumors , whose morbidity and mortality rates at the top of oral cancer. Its high degree of malignant, strong infiltration, lymph node metastasis early, poor prognosis and secondary deformity after surgery effect the quality of life of patients with OSCC serious.Cytokeratin is one of the major components, in the cells have important physiological functions, which has closely relationship with several human solid tumor growth, invasion and metastasis. We used immunohistochemical SP techniques to detect the expression of ck17 and ck19 in OSCC and normal oral tissue ; explored its relationship with the clinicopathological features and biological behavior of OSCC.Materials and Methods:1. 10 normal oral tissue and 78 OSCC were selected. Information of these cases was collected including age, gender,, lymph node metastasis and clinical stage.2. Using immunohistochemical SP staining, we examined ck17 and ck19 expression in paraffin slices of 78 oral squamous cell carcinomas. As control, expression of these proteins in 10 normal oral tissue were also detected.3. Difference of ck17 and ck19 were analyzed in normal oral tissue and oral squamous cell carcinomas, Relationships of these protein expression with clinicopathological parameter were analyzed in OSCC, and so were relationship of protein ck17 expression with ck19 protein expression.Result:1. The expression of ck17 in OSCC major located in tumor bolt,the forefront of tumors extended or the edge area , and occasional expression located cell nucleus. The positive expression rate of ck17 is 47.4% in OSCC, but negative expression in normal oral tissue. The positive (+) were 28 cases (35.9%); the stronger positive (+ + ) were 9 cases (11.5%). The over-expression of ck17 had positive correlation with clinical stage ,histological differentiation and lymph node metastasis of OSCC (P<0.05) by the statistical analysis. Expression of ck17 was not correlated with patient age, gender, tumor site and size.(P>0.05).2.The positive expression of ck19 brown staining parts located in cytoplasm and membrane of oral cancer cells. There were 48 cases positive expression in 78 cases. The positive expression rate of ck19 is 61.5%. The positive (+) were 36 cases (46.2%); the stronger positive (+ + ) were 12 cases (15.4%), but negative expression in normal oral tissue. The over-expression of ck19 had positive correlation with clinical stage, histological differentiation and lymph node metastasis of OSCC (P<0.05). Expression of ck19 was not correlated with patient age, gender, tumor site, size(P>0.05).3.The relationship between ck17 and ck19 expression There were 37 cases ck17 positive expression, and there were 16 cases ck19 positive expression in those. The correlation coefficient was 0.392 by Pearson's correlation analysis. Positive relationship between ck17 and ck19 expression was observed in oral squamous cell carcinoma (P<0.05).Conclusion:Expression of ck17 and ck19 were significantly associated with tumor progression and development in OSCC, which may serve as a significant predicting factor of clinical stage andd trend of metastasis in OSCC. Over-expression of ck17 and ck19 may promote each other in unknown signal transduction pathway.
