Objective:TO explore clinical efficacy and safety of specific immunotherapy with sublingual dust mite on children with allergic asthma,To obtain a wider range of clinical applications, which will help to control asthma.Methods:Fifty children with asthma were randomly divided into two groups, The active group 30 cases(inhalation of dust mite drops + corticosteroid) and The control group 25 cases (inhalation of corticosteroid ) , All the patients were observed by 1 year and Symptom scores, acute attack of asthma times and dates and Extent ,ICS dosage,PEFR (PEFam and PEFpm ) and pulmonary function (The predicted value of FEV1, PEF,υ75,υ50 and MMEF) were compared between the two groups. Observation methods : The active group and the control group must experience 4 weeks observation at base period .The active group(SLIT+ corticosteroid), The control group (inhalation of corticosteroid) .All the patients were observed by 1 year, during the study , asthma diary was used for symptoms scores and acute attack of asthma times and dates and Extent ,and ICS dosage andPEFam and PEFpm of everyday,toPEFvariation=∣(PEFam-PEFp/(PEFam+PEFpm)∣×2×100 %。before and after our experiment. MasterScope children's spirometry was used for the pulmonary function assessment(index: FEVl,PEF,FEF 50,FEF 75,MMEF account for predicted value).Results:A total of 55subjects were randomized and 50 subjects(29 in the active group,21 in the control group) completed the whole study . 5 subjects dropout because of poor compliance. two groups were comparable at baseline in all clinical parameters. The mean daily symptom scores in 4-weekly began to diverge at the 36th week with the active group showing a significant lower scores(0.28±0.05)than the control group (0.39±0.06)(p<0.05)till the end of study .there were significant differences in the acute attack of asthma frequence and dates and extent(qing zhong du)between in 1year the active group(1.50±0.96,9.5±7.8,60%,26.67%and the control group(2.60±1.06;(19.2±9.5;80.35%;16.07%)。The mean daily ICS dosage in 4-weekly began to diverge at the 40th week with the active group (112±12)μg showing a significant lower dosage than the control group (148±14)μg(p<0.05)till the end of study. PEF variation between 1-28 th week active group(2.6±2.0),29-56 th week(1.1±1),there were significant obvious diference in two period. there were no obvious diference in the control group between 1-28 th week(2.7±1.9)than 29-56 th . 1-28 th there were no obvious diference between two groups.while29-56 th active group were obviously lower than the control group.Before experiment, there was no significant diference between two groups. After 1 yearof experiment percent of predicted FEF50 MMEF in active group was significantly higher than that in control group But there was no obvious difference in percent of predictedFEV1,PEF,FEF75, between two groups.Conclusion:Specific immunotherapy with sublingual dust mite can significantly improved asthma symptom scores,reduce the number of asthma attack days,Shorten attack duration days and Reduce the severity of asthma; SLIT can significantly reduce the dosage of ICS in children with asthma; Reduce the variation rate of PEF and improve small airway in children. This method is portable, safe, It's worth Promoting in clinical practice.
