Effects Of Propofol On Lung Injury Induced By Hepatic Ischemia Reperfusion In Rats

[Objective]Hepatic ischemia reperfusion (I/R) model in rats was established and Propofol injection was used as an intervention for lung injury. The aim of this study was to detect the expression of TNF-a and NF-KB in hepatic tissue and explore protective effects of Propofol in hepatic tissue in rats.[Methods]48 Sprague-Dawley (SD) rats were randomly divided into 4 groups and in each group there were 16 rats:(1) sham-operated group (Sham group,6 rats), only expose hepatic pedicle without any intervention; (2) ischemia group (I group,6 rats), expose and clamp hepatic pedicle, after 30min sacrifice the rats; (3) ischemic reperfusion model group(HI/R group,18 rats), clump hepatic pedicle for 30min before reperfusion, an injection of 0.5ml physiologic saline in femoral vein were applied; (4) Propofol intervention group (Pro group.18 rats), clump the hepatic pedicle for 30min, reperfusion before injection of Propofol (2mg/kg) in femoral vein (diluted in 0.5 ml of physiologic saline). All animals were sacrificed by 30.60 and 120 minite point in (3) and (4) groups and then lung tissue were obtained.The pathological changes of lung were studied. Wet and dry weight of lung tissue was measured to calculate the wet to dry weight ratio (W/D). The expression of TNF-αand NF-κB were measured by the histochemical staining. Results were analyzed using SPSS 16.0 statistical software. Results of data were expressed using mean±tandard deviation(x±s). The comparison group use paired t- test and group comparison use one-way ANOVA. Take a=0.05 level test. P<0.05 indicate statistical significance.[Results]1. No obvious change has been observed in Sham-operated group duo to free clamping of hepatic pedicle so that no hepatic ischemia took place. Lung tissue looks pale pink under naked eye and no congestion, no edema, no hemorrhage, nor necrosis under microscope. Lung tissue gives out a dark pink color. a little foamy liquid has been observed in ischemia group. Volume of lung tissue is enlarged in hepatic ischemia reperfusion group, there's visible edema, congestion and bleeding under microscope whereas propofol pretreatment group show a prominent lessened pathological change in comparison with ischemia-reperfusion group.2.Lung W/D results show:HI/R group at each time point W/D ratios were higher than the corresponding time Pro group, sham operation group andⅠgroup W/ D ratio (P<0.01). Pro group at different time points W/D ratio is less than HI/R outside the corresponding time point (P<0.01), andⅠGroup W/D ratio of the difference was not statistically significant (P> 0.05). Pro 30min and Pro 60min with the sham group showed no statistical difference (P> 0.05).3. Immunohistochemical staining showed that:(1) TNF-αlevels measured inⅠgroup compared with the sham group, the difference was not statistically significant (P> 0.05). (2) whether HI/R group or the Pro group, the lung tissue from reperfusion 30min TNF-αbegan to increase, but its value in the HI/R group until 120min still rising status, both in the Pro group, the values 60min after reperfusion were peaked. I set the measured value of TNF-αlevel higher than sham operation group, but the comparison is not statistically significant. In the Pro group HI/R group compared with the corresponding time point Pro levels were lower than in lung TNF-αHI/R group (P>0.01). (3) TNF-αcontent values measured between groups, except in Pro 30min, Pro 120min and I was no significant difference between groups outside (P>0.05), HI/R group and pro60min group TNF-αcontent value measured at each time point group were higher thanⅠ(P>0.05). (4) TNF-αlevel measured values between groups, Pro group at each time point measured were less than HI/R group. Pro Group TNF-αcontent measured value. Pro 60min and Pro 120min comparison with the sham group was significant (P>0.05), and I in Pro 60min group was statistically significant (P> 0.05). Pro 30min and Pro 120min measured value compared with the results I was no significant (P<0.05), and Pro 30min content of lung tissue measured TNF-αvalue compared with the sham group had no statistical significance. (5) HI/R group compared, HI/R 60min and HI/R 120min measured values of lung tissue and HI/ R 30min TNF-a statistically significant difference. The comparison results between HI/R 60min and HI/R 120min no statistical significance. (6)Pro group compared with the results measured at three time points the difference was not statistically significant.Ischemia-reperfusion model of liver tissue from TNF-a level in a reperfusion began to increase 30min,60min to reach peak reperfusion, and then decreased gradually. Each time point of liver ischemia-reperfusion model of tissue TNF-a levels to be significantly higher than sham operation group, the difference between the two groups was significant (P<0.05). Propofol in the intervention group liver tissue changes of TNF-a level of a basic model with ischemia-reperfusion group, but each time a group of liver TNF-αlevels to be significantly lower than the R group between the two groups significant difference (P<005).4. Results of NF-κB shows:(1) NF-κB in I group was higher than in sham group, but lower than in HI/R group and Pro group. And the HI/R group of three time points were analyzed statistically, There were significant differences in comparison (P<0.01). (2) HI/R group of a NF-κB lung tissue was higher than sham operation group, I group and the Pro group. Its content began to increase from reperfusion 30min.60min to reach peak reperfusion, and then decreased gradually. (3) Pro lung tissue at each time point measured NF-κB were significantly lower than that of a NF-κB HI/R group at corresponding time points, the group difference was significant (P<0.01), and the trend with all time points ischemia-reperfusion group the same. (4) HI/R group compared.60min and 120min NF-κB lung tissue measured values of a NF-κB compared with HI/R30min significant difference (P<0.01), while the comparison between 60min and 120min no statistical Significance (P> 0.05). (5) Pro group compared with the results measured at three time points showed no statistically significant (P>0.05).[Conclusions]Hepatic ischemia-reperfusion causes lung injury.Propofol can help to protect liver from ischemia reperfusion in rats.
