Objective To evaluate the effects of propofol or remifentanil on hepatic cell apoptosis induced by intestine ischemia-reperfusion in rats.Methods Seventy-two healthy Wistar rats didn't discriminated female and male ,weighing 200-250g were randomly divided into 4 groups as follows (n=18):①sham-operated control group (Sham group):do not blocking the superior mesenteric artery, the other processes are coincide with intestine ischemia-reperfusion group.②intestine ischemia-reperfusion group (IIR group): ten minutes before ischemia constant intravenous infusion saline 10ml·kg-1·h-1 till executed.③propofol group (P group): ten minutes before ischemia give loading dose propofol 5.0mg/kg , then constant intravenous infusion propofol 20mg·kg-1·h-1 till executed.④remifentanil group (R group): ten minutes before ischemia constant intravenous infusion remifentanil 1μg·kg-1·min-1 till executed. Each group divided 3 time phases at 1h, 3h, 6h after reperfusion. The model of hepatic injury induced by intestinal ischemia-reperfusion was established by blocking the superior mesenteric artery one hour with clap. The left hepatic lobes were sampled at the end of reperfusion. Light microscopy and electron microscopy were performed to verify the morphologic and ultrastructural changes of hepatic cell apoptosis. Expression of Bcl-2 and Bax protein in hepatic was detected with immunohistochemistry and MIAS-2000.The apoptosis index (AI) of hepatic cell was measured by terminal deoxynucleotidy transferase mediated dUTP nick end labeling (TUNEL) method.Results①The result of light microscopy and electron microscopy: compared with Sham group, IIR group showed great changes. The pathological lesion were lighter in P,R group than in IIR group.②The change of AI: Apoptosis index of hepatic cell were significantly higher in IIR,P,R groups than in Sham group at each point (P<0.05). AI were significantly lower in P,R groups than in IIR group at each time point (P<0.05). The result of AI between P group and R group at each time point didn't have meaning of statistics (P>0.05). In IIR,P,R groups, AI of hepatic cell had significant difference between 3 time points (P<0.05). As reperfusion time prolonged, AI of hepatic cell in IIR,P,R groups were increased, highest at hour 6 (P<0.05).③The optical density of Bcl-2, Bax and Bcl-2/Bax ratio: Bcl-2, Bax protein optical density were significantly higher in IIR,P,R groups than in Sham group at each time point (P<0.05); Bcl-2 protein optical density were significantly higher, while Bax protein optical density were significantly lower in P,R groups than in IIR group at each time point (P<0.05). Bcl-2/Bax ratio in IIR group was lower than in Sham group at each time point (P<0.05), while it were higher in P,R groups than in Sham group at each time group (P<0.05);Bcl-2/Bax ratio in P,R groups were higher than in IIR group at each time point(P<0.05). The optical density of Bcl-2, Bax , Bcl-2/Bax ratio between P group and R group at each time point didn't have meaning of statistics (P>0.05). Optical density of Bcl-2, Bax protein and Bcl-2/Bax ratio had not on significant difference between 3 time points in 4 groups(P>0.05).Conclusions①Apoptosis of hepatic in injury induced by intestine ischemia-reprefusion.②Propofol and Remifentanil can relieve the liver cellular apoptosis by intestinal ischemia-reperfusion in rats, which concerned with up-regulating Bcl-2 and inhibiting Bax protein expression.③Propofol and Remifentanil didn't have meaning of statictics in protect to hepatic injury by intestine ischemia-reperfusion.
