Objective:To investigate the association between chronic periodontitis and coronary heart disease with gene polymorphsims of tumor necrosis factorA(TNFA) and porphyromonas gingivalis(P.g). Methods:TNFA gene polymorphsims at position-857,-863and-1031 were determined by PCR-RFLP with genomic DNA extracted from buccal mucosa in 115 Chronic periodontitis(CP) patients,107 Coronary heart disease(CHD) patients,78 CP companied CHD(CP+CHD) patients and 115 Healthy controls(HC); Subgingival plaque samples and coronory atherosclerotic plaque were harvested from a total 38 patients with periodontitis who scheduled for coronary artery bypass surgery, to test P.g respectively.The differences of genotypes and the variation of allele frequences and the detection rate of P.g were analyzed by Chi-square test. The copies of P.g in subgingival plaque and in coronary atherosclerosis plaque were tested by RT-qPCR. Results:There was no statistical difference about the distribution of TNFA-857 and TNFA-1031 genotype among the four groups(both of their P>0.05).But the frequences of TNFA-863CC genotype and allele C in CHD group and CP+CHD group were significantly higher than that in HC group.Forthmore, in CP+CHD group the frequences of TNFA-863CC genotype and allele C in male patients were significantly higher than that in female patients(both of their P=0.000, OR=4.292 and 5.562 respectively,95%CI: 1.884-9.777 and 3.185-9.713respectively); The detection rate of P.g in subgingival plaque and in coronary atherosclerosis plaque was 76.32% and 68.42% respectively; the concordant presence of P.g in subgingival plaques and in coronary atherosclerosis plaques in the same patient was 52.63%.The P.g detection rate of susceptible gene carrier was little higher than that of non-carier, but there was no statistical difference between carrier and noncarrier; The copies of P.g in two simples are significantly different. Conclusion: TNFA-863CC genotype and allele C may be associated with CHD, they also may be the susceptible gene to CP companied CHD patients, especially in male patients; P.g have lived in coronary artary tissue and it was associated with coronary heart disease.Furthermore, the susceptibal gene may be associated with detection rate of P.g, the susceptibal gene carriers may be more susceptibal than non-carriers.
