Objective: The aim of this study was to assess the validity of glycated haemoglobin (HbA1c) and/or fasting plasma glucose (FPG) as screening tests for the early detection of diabetes in high-risk subjects.Methods: A total of 202 subjects underwent an oral glucose tolerance test(OGTT)for screening diabetes. 174 subjects (110 male and 64 female) intact data with risk factors for diabetes were included. One group of 70 patients, male 42, female 28; aged 38-77 years, mean age 56.93±8.50. Control group of 104 people, 68 male, 36 female; aged 36-80 years, mean age 55.11±9.68 years. All subjects underwent a 75-g oral glucose tolerance test, HbA1c measurement and heart coronary artery visualization examine or pars cervical blood vessels color Doppler examine.Results:Using 1999 WHO criteria,70 had diabetes.110 had no diabetes. Using a receiver operating characteristic curve(ROC curve),The optimal cut-point of HbA1c related to diabetes diagnosed by OGTT was 5.85%,which was associated with a sensitivity and specificity of 84.3% and 71.2% respectively. Area under the curve was 0.828.The optimal cut-point of FPG related to diabetes diagnosed by OGTT was 5.66mmol/L that was associated with a sensitivity and specificity of 72.9% and 86.5% respectively. Area under the curve was 0.848. HbA1c and FBG are linear dependence. OGTT2hPG and HbA1c have strong dependability.Conclusions: 1,HbA1c is more sensitive than FPG in diabetes diagnose. HbA1c might be a more suitable screening tool for identifying high-risk individuals with diabetes at an early stage. 2,When the FPG in the 5.425-6.1mmol/L and HbA1c≥6.1%, it is possible postprandial hyperglycemia, suggest further lines OGTT. 3,If HbA1c is above 5.85%,one should be have OGTT clear whether the high glucose.
