Clinical Analysis Of Combined Therapy Of Intraoperative Hepatic Artery Chemoembolization And Microwave Coagulation For Unresectable Liver Tumors

Purpose To study the clinical outcomes of combined therapy of intraoperative hepatic artery chemoembolization and microwave coagulation for the liver tumors unresectable due to poor hepatic function,severe liver cirrhosis or multiple tumors.Method To retrospectively analyze the curative effect, complications, survival rates of 11 cases with unresectable liver tumors undergoing intraoperative hepatic artery chemoembolization and microwave coagulation in our hospital from April 2005 to March 2009 .Result Among the 11 patients treated with combined therapy of intraoperative hepatic artery chemoembolization and microwave coagulation, there were 5 patients who underwent hepatic artery embolization chemotherapy via chemotherapy pump for one, and 2 cases underwent the above treatment for 3 times. After one week after the therapy, 9 cases with preoperative AFP levels higher than normal indicated decreased AFP levels after the treatment . Color-Doppler ultrasound showed that the tumors were shrinked, no blood supply signal was found. Abdominal plain X-ray film showed lipiodol deposition in the tumor areas. After the treatment, ALT and AST were increased significantly, hower ,1 to 2 weeks later they could return to normal level. 1,2,3 year survival rate was 10/11(90.9%), 7/11(63.6%), 2/11 (18.2%). 10 cases tumor size>5cm, the survival rate of 1,2,3-year was 9/10(90%), 6/10(60%),1/10(10%). There are still 5 patients alive. The survival period after operation were 51,42,14,24,34months respectively. Conclusion HCC patients with poor liver function, severe liver cirrhosis, multiple tumers are not able to tolerate liver resection, the intraoperative hepatic artery chemoembolization combined with microwave coagulation is an effective method with few complications.
