Objective: To compare the levels of expression of phagocytic NADPH oxidase p22phox subunit,MDA,TAOC of venous blood in patients with essential hypertension,and to study their correlation with LVMI,expecting to explore the role of oxidative stress resulted by the increased activity of phagocytic NADPH oxidase in left ventricular remodeling. Methods:To collect 61 patients with EH having no pills of hypotensive drug,and 18 objects with normal blood pressure of our hospital.Registering their name,sex,age, height, weight,timing of hypertension and blood pressure, pulse pressure, to calculate BSA and BMI.All the objects should receive UCG. Writing down IVST,LVPWT,LVEDd,LVEDs,EF.(LVM,g)=0.8×1.04(IVST+LVPWT+LVEDd)3 ? LVEDd3 +0.6,BSA(m2)=0.0061×H(cm)+0.0128×W(kg)-0.1529.LVMI=LVM/BSA.At the same time,TC,TG,LDL-C,HDL-C,BUN,CR,UA,FBG,hs-CRP,MDA,TAOC were detected. Expression of p22phox mRNA was detected by semi-quantitative reverse transcription polymerase chain reaction(RT-PCR).TAOC was detected by ABTS,and MDA was detected by ELISA.All the objects were divided into EH group and control group;all the EH patients were divided into 3 groups based on bood pressure,that was EH1,EH2,EH3.Based on LVMI,all the patients were divided into sole EH group and EH with LVH group.Results:1.EH group had higher levels of SBP,DBP,TC,UA,IVST,LVPWT than control group,the difference had statistical significance(P=0.048,0.026,0.018,0.028,0.042,0.047;P <0.05);and EH with LVH group had a significantly higher level of TC than sole EH group(P<0.05).2.RT-PCR demonstrated the expression of phagocytic NADPH oxidase p22phox of EH was stronger than that of control group(P=0.004,P<0.05),and EH group had a high level of MDA and lower level of TAOC.( P=0.005,0.000,P<0.05).3.Expression of phagocytic NADPH oxidase p22phox of EH with LVH was stronger than that of sole EH group(P=0.020,P<0.05),and MDA of EH with LVH was higher than that of sole EH(P=0.025,P<0.05).TAOC of the former had a lower level (P=0.009,P<0.05).4.Both EH2,EH3 had high levels of p22phox than EH1(P=0.007,0.001,P<0.05),but difference between EH2 and EH3 was not significant(P=0.667,P>0.05);MDA of EH3 was significantly higher than EH1 or EH2(P=0.010,0.017,P<0.05),but there was no significant difference between EH1 and EH2(P=0.300,P>0.05).TAOC of different groups of EH had no significant difference(P>0.05).5.LVMI had positive correlation with expression of p22phox and MDA,(r=0.707,0.591,P<0.05),it increased with the higher expression of p22phox and MDA;and LVMI had a negative correlation with TAOC,r=-0.245,(P<0.05).Correlation between LVMI and p22phox was the best.6.SBP,PP,UA , TC were the positive correlation factors of expression of p22phox(r=0.251,0.261,0.263,0.282,P<0.05);p22phox had a trend of positive correlation with hs-CRP,but correlation coefficient had no statistical significance(r=0.151,P>0.05);and it had no obvious correlation with DBP,BUN,CR,TG,LDL,HDL(r=0.104,-0.060,-0.129,0.083,0.019,-0.007,P>0.05).Conclusions:1.Expression of phagocytic NADPH oxidase p22phox subunit,serumal level of MDA in patients with EH were higher than that of control group,and the latter had a lower level of TAOC,which showed that oxidative stress increased in hypertension.These three factors could evaluate the degree of oxidative stress well. 2.With higher level of expression of p22phoxmRNA and MDA,left ventricular remolding aggravated,which showed that oxidative stress had relationship with left ventricular remodeling,and it indicated that oxidative stress participated in hypertension and left ventricular remodeling.3.Correlation between LVMI and p22phoxmRNA was better than MDA and TAOC,which showed among all the factors that influenced oxidative stress,increased activity of NADPH oxidase could play an important role in the left ventricular remodeling.
