Objectives:The study is aimed to explore whether serum HMGB1(high-mobility group protein B1) levels is associated with left ventricular remodeling and diastolic dysfunction in essential hypertension.Methods:A total of48hypertensive patients were enrolled in this study.The control group included30people who were examined in our medical centers.ECG,Echocardiography,serum HMGB1and TNF-alpha levels were determined in all of the subjects.Serum HMGB1and TNF-alpha levels were determined by the enzyme-linked immuno sorbent assay kit.The following ECG parameters were also determined:QTd, QTcd and Rv5+Sv1.Echocardiography parameters included LVMI,RWT, E/A,Em and Em/Am.Results:1.Hypertensive patients showed significantly higher the serum levels of HMGB1and TNF-alpha comparing with the control group (P<0.01).2.Compared to the control group,the values of LVMI,RWT in Hypertensive patients were higher(P<0.01),while the values of E/A, Em, Em/Am in Hypertensive patients were lower(P<0.01). 3.Single-factor correlation analysis showed that:In all subjects, serum HMGB1levels were positively correlated with systolic blood pressure,diastolic blood pressure,pulse pressure,the TNF-alpha levels, QTcd, left ventricular voltage,LVMI(P<0.01), and negatively correlated with Em,Em/Am,E/A(P<0.01).4. The stepwise regression analysis where LVMI was regarded as the dependent variable showed that systolic blood pressure,serum HMGB1and TNF-alpha levels were independent factors to LVMI.The multiple linear regression equation was concluded in this form:LVMI=10.495+1.494TNF-α+0.028HMGB1+0.177SBP (F=49.05,P<0.01)Conclusions:1.Hypertensive patients showed significantly higher the serum levels of HMGB1.2. The serum levels of HMGB1was correlated with left ventricular diastolic dysfunction in essential hypertension.3. Serum HMGB1levels were positively correlated with LVMI in essential hypertension.
