Objects: To detect the expression of the FHIT, P14ARF and CASPASE-3 protein in endometrial adenocarcinoma (EC), endometrial atypical hyperplasia (EIN) and the normal endometrium, and examine the possible roles and relationships of FHIT, P14ARF and CASPASE-3 in the genesis and progression of endometrial carcinoma.Methods: Using immunohistochemistry SP method to detecte the FHIT, P14ARF and CASPASE-3 protein expression in 60 cases of endometrial adenocarcinoma, 30 cases of endometrial atypical hyperplasia, 30 cases of normal endometrial tissue. And the expressions of FHIT, P14ARF and CASPASE-3 in endometrial adenocarcinoma tissues and surgical - pathologic staging, histologic grade, myometrial invasion, lymph node metastases and other clinicopathological parameters are analyzed to examine the relevance of FHIT, P14ARF and CASPASE-3.Results:The positive expression rates of FHIT in endometrial adenocarcinoma ,endometrial atypical hyperplasia and normal endometrium were 53.3%, 63.3%, 100% respectively. The positive expression rates of FHIT in endometrial adenocarcinoma and endometrial atypical hyperplasia is significantly lower than that in normal endometrium , The difference was statistically significant (P <0.01). The differece of the positive expression of FHIT between endometrial atypical hyperplasia and endometrial adenocarcinoma was not statistically significant (p> 0.05).The positive expression rates of P14ARF in endometrial adenocarcinoma ,endometrial atypical hyperplasia and normal endometrium were 56.7%, 73.3%, 9.00% respectively.The positive expression rate of P14ARF in endometrial adenocarcinoma was significantly lower than that in normal endometrium, The difference was statistically significant (P <0.01) (P <0.01). The differences of the positive expression of P14ARF between endometrial atypical hyperplasia and endometrial adenocarcinoma, normal endometrium were not statistically significant (P> 0.05).The positive expression rates of CASPASE-3 in endometrial adenocarcinoma and endometrial atypical hyperplasia and normal endometrium were 51.7%, 70.0%, 86.7% respectively. The positive expression rate of endometrial adenocarcinoma CASPASE-3 was significantly lower than that in normal endometrium. The difference was statistically significant (P <0.01). The differences of the positive expression of CASPASE-3 between endometrial atypical hyperplasia and endometrial adenocarcinoma, normal endometrium were not statistically significant (P> 0.05).The positive expression of FHIT was associated with the surgical - pathologic staging of endometrial adenocarcinoma, organizational grade, myometrial invasion and lymph node metastasis (P <0.05). The positive expressions of P14ARF and CASPASE-3 were only related to the organizational grade of endometrial adenocarcinoma (P <0.01), and were not related to surgical - pathologic staging, myometrial invasion, and lymph node metastasis. The positive expressions of FHIT, P14ARF and CASPASE-3 protein were not related to age. The positive expressions between FHIT and P14ARF, FHIT and CASPASE-3, P14ARF and CASPASE-3 protein were of significant positive correlation in endometrial adenocarcinoma (rs = 0.396,0.566,0.433, P <0.01).Conclusion: The expression loss of FHIT protein may be an early event in progression of endometrial adenocarcinoma , and is related to the occurrence of endometrial adenocarcinoma, development and invasion, metastasis and prognosis. The expression loss of P14ARF and CASPASE-3 may be related to the occurrence of endometrial adenocarcinoma, and may be an important factor in endometrial cells malignant differentiation. FHIT, P14ARF and CASPASE-3 proteins may take part in the carcinogenesis and progression of endometrial adenocarcinoma together in complementary or cooperative manner. The combined detection of FHIT, P14ARF and CASPASE-3 protein has certain reference value in early diagnosis of endometrial adenocarcinoma and endometrial adenocarcinoma cell differentiation, lymph node metastasis and prognosis.
