Objective:Compare the differences of PSA and side effects of maximum androgen blockage(MAB) with surgical castration, and evaluate the prospects and effects of the treatment for advanced prostate cancer using bicalutamide and goserelin.Methods:43 patients with advanced prostate cancer were divided into two groups. MAB group used Goserelin and Bicalutamide for Anti-androgen therapy, and surgical castration group treated by bilateral orchiectomy. The number of cases were 20 cases and 23 cases.The whole cases tested serum levels of prostate specific antigen(PSA) before surgery (before treatment) and a week (one week after treatment), 1,3,6,9,12months after castration.Results:1. MAB group after 6 months of treatment,18 patients (90.0%) patients with serum prostate specific antigen PSA<1ng/ml, and fell to the lowest PSA value 9 months latter.2. Surgical castration group after 6 months of treatment,20 patients (87.0%) patients with serum prostate specific antigen PSA<1ng/ml, down to the lowest PSA value 6 months latter.3. MAB Group's PSA declines faster than the surgical castration group, in the end of the first month that serum levels of PSA is less than 4ng/ml.Compare to surgical castration,MAB can delay 3 months about PSA's rising on average.Conclusion:Using Goserelin and Bicalutamide for maximum androgen blockage(MAB) for advanced prostate cancer better than surgical castration alone, and castration's side effects that compared with surgery alone have statistically significant, and surgical castration may cause psychological problems in patients. Thus, maximum androgen blockage for advanced prostate cancer will be a first-line endocrine therapy options.
