Objective:To explore the role of spiral artery transforming defection and the ultrastructure alteration of the spiral artery,extravillous trophoblast in early spontaneous abortion through observing the condition of spiral artery transformation and the ultrastructure of the spiral artery and extravillous trophoblast of patients with early spontaneous abortion and normal early gestation.Method:Choosen 30 cases of early spontaneous abortion were studied, which were divided into group A (embryo survival group) 15 cases and group B (embryonic loss group) 15 cases.And 25cases of normal early gestation women were selected as normal groups.Immunohistochemistry was performed to observe the density of invaded extravillous trophoblast in decidua of early spontaneous abortion and normal early gestation. Electron microscope was used to observe the ultrastructure of the spiral artery and extravillous trophoblast of patients with early spontaneous abortion and normal early gestation.Results:1. The density of extravillous trophoblast which invade the decidual in normal early gestation group,early spontaneous abortion group A and early spontaneous abortion group B is (15.71±4.33) (11.55±4.40),(10.11±5.32) respectively.The difference between normal early gestation group and early spontaneous abortion group A normal early gestation group and early spontaneous abortion group B had statistical significance respectively (P<0.05).But there was no statistical significance between early spontaneous abortion group A and early spontaneous abortion group (P>0.05).2. There was extravillous trophoblast in 49.30% lumen of sp iral arteries in normal pregnancy, 30.22% was seen in early spontaneous abortion group A, while 22.41% was seen in early spontaneous abortion group B.The difference between normal early gestation group and early spontaneous abortion group A normal early gestation group and early spontaneous abortion group B,early spontaneous abortion group A and early spontaneous abortion group B had statistical significance respectively (P<0.05).3.The average lumen diameter of the spiral arteries in the decidual segment in normal early gestation group,early spontaneous abortion group A and early spontaneous abortion group B is(160.25±21.36)μm,(136.67±33.14)μm,(99.67±24.78)μmrespectively.The difference between normal early gestation group and early spontaneous abortion group A,normal early gestation group and early spontaneous abortion group B,early spontaneous abortion group A and early spontaneous abortion group B had statistical significance respectively (P<0.05).; The thickness of spiral arteries wall in normal early gestation group,early spontaneous abortion group A and early spontaneous abortion group B is (81.16±11.51)μm,(93.08±17.07)μm,(100.77±16.17)μm respectively.The difference between normal early gestation group and early spontaneous abortion group A,normal early gestation group and early spontaneous abortion group B,early spontaneous abortion group A and early spontaneous abortion group B had statistical significance respectively (P<0.05).4. In early spontaneous abortion group the ultrastructure of extravillous trophoblas which invade the decidua and spiral arterywas abnormal,such as mitochondria swell,endoplasmic reticulum expand.Conclusion:1.The defection of spiral artery transformation is concerned with the occurrence of early spontaneous abortion.2.The abnormal ultrastructure alternation of the spiral artery and extravillous trophobast has close relation with the invading ability of extravillous trophoblast and the physiological change of the spiral artery, it may be one of the important factors inducing the occurrence of early spontaneous abortion.
