ObjectivePartâ… : To establish a new enzyme-linked immunosorbent assay (ELISA) for Helicobacter pylori (Hp) urease B (ureB) and to investigate infection of Hp in patients with digestive system diseases.partâ…¡: To investigate the role of Helicobacter pylori (Hp) infection in patients with idiopathic thrombocytopenic purpura (ITP),and to explore the therapeutic effect of Hp eradication.MethodsPartâ… : The ELISA method was established by using two monoclonal antibodies against Hp ureB. ELISA was performed in 60 patients who had upper gastrointestinal symptoms and underwent gastroscopy. 14C- breath test,rapid urea test (RUT) and polymerase chain reaction (PCR) were also performed in these patients. The combination of 14C- breath test and RUT was used as a gold standard for detection of current infections with Hp.partâ…¡: ELISA for Hp urease B (ureB) was performed in 45 patients with ITP as well as 32 normal individuals as controls. Hp-positive ITP patients were treated using Hp eradication procedure,and their platelet counts were compared with those of Hp-negative patients after treatment.ResultsPartâ… : Compared to the gold standard,the sensitivity and specificity of this ELISA for Hp ureB in plasma were 91.7% and 83.3%,respectively. The positive rate was 78.3%,the positive and negative predictive values of this test were 93.6% and 76.9%,respectively,and the accuracy was 90%. The sensitivity and specificity of this ELISA for Hp ureB in stool were 95.8% and 83.3%,respectively. The positive rate was 81.7%,the positive and negative predictive values of this test were 93.9% and 90.9%,respectively,and the accuracy was 93.3%.partâ…¡: The Hp-positive infection rates of ITP group and the normal control group were 66.7% and 43.8%,respectively (P <0.05). In 25 patients who received Hp eradication treatment,21 became Hp-negative,and their platelet counts were significantly increased in comparision with Hp-negative ITP patients.ConclusionPartâ… : The ELISA for Hp ureB in plasma and stool is a sensitive and specific method for rapid diagnosis of Hp current infection.partâ…¡: The frequency of Hp infection in ITP patients was higher than that in normal individuals. Part of chronic ITP patients whose Hp infection was successfully eradicated showed increase in platelet counts. These results suggest that Hp infection play an important role in ITP pathogenesis.
