Objective:Discussing the risk factors and prognostic factors Of the liver cirrhosis complicated with spontaneous bacterial peritonitis. Methods:We collected 187 cases of patients with cirrhosis in Jincheng Coal General Hospital from January 1,2000 to December 31,2009, of which 87 cases occurred SBP. We analysized the risk factors, laboratory findings and outcome of them in case-control study. Statistical methods applied SPSS 13 software. The x2 test is used to compare count data. Two independent samples t test was used to compare the mean. We used Logistic regression analysis in multiariate analysis. Test levelα= 0.05.Results:1.The patients that their serum total bilirubin was greater than 51umol/L, their ascitic fluid total protein was less than 10g/L,.and they had gastrointestinal bleeding were significantly different between the SBP group and the non-SBP group (χ2 values were 20.950,14.713,8.142; P <0.005).2. The patients that their peripheral blood neutrophils, ascites white blood cells and ascites neutrophil counts were significantly difference between the SBP group and tne non-SBP group(χ2 values were 83.383,91.324,121.336;P<0.01).3.In 17 factors of affecting prognosis, PTA, Child-pugh classification, serum bilirubin, serum creatinine, serum sodium, complicated by gastrointestinal hemorrhage, hepatic encephalopathy, liver and kidney Syndrome are statistically significant (t value orχ2 values were:-7.413,21.184,-10.121,-5.018,10.926,19.166,10.864,44.830;P <0.05) in single-factor analysis.4.On the single factor analysis of significant variables,the serum sodium reduction was significantly different between the cure improved group and the infective death group of SBP in Logistic regression analysis (B value of-0.352; P value of 0.04; OR value of 0.703). SBP integrated concurrent liver and kidney Disease mortality was significantly higher than the unnecessary complications (B value of 3.327; P value of 0.049; OR value of 27.876). Severe jaundice is another factor of affecting the prognosis of SBP (B value of 0.035; P value of 0.009; OR value of 1.036).Conclusion:1. Hyperbilirubinemia, ascites total protein content decreased and gastrointestinal bleeding are risk factors of cirrhosis complicated with SBP.2. Neutrophil percentage, ascites white blood cell count and ascites polymorphonuclear cell count have important role in the diagnosis of SBP. 3. Serum sodium is a protective factor in the liver cirrhosis complicated with spontaneous bacterial peritonitis.4. severe jaundice and hepatorenal syndrome are independent risk factors.of affecting the prognosis in the liver cirrhosis complicated with spontaneous bacterial peritonitis.
