ObjectiveTo study differences between pyogenic liver abscesses of biliary origin (PLAB) and cryptogenic origin (PLAC) in the clinical features and treatment outcome.MethodsWe studied 205 cases of liver abscesses admitted to Fujian Province Hospital between 2002 and 2009. The clinical data and treatment outcome of patients were recorded and reviewed.58 were classified into the biliary group and 92 were classified into the cryptogenic group.ResultsPatients in PLAB group had a higher proportion of male patients (58.6%) and an advanced age (60;38±12.6 years) compared to those with pyogenic liver abscesses of cryptogenic orign.Jaundice and co-existing disease of malignant tumor are more prevalent in pyogenic liver abscesses of biliary,which has no correlation with prognosis contrasted to single use of antibiotics.Escherichia coli and co-existing disease of biliary disorder were more prevalent in the biliary group while Klebsiella pneumoniae and diabetes mellitus were more prevalent in the cryptogenic group. Most of liver abscesses are single and the right-sides in cryptogenic group.There is no difference between PLAB and PLAC in the results of different treatment outcome and hospital stay.ConclusionJaundice and E.coli were more prevalent in pyogenic liver abscesses of biliary while co-existing disease of malignant tumor and Klebsiella pneumoniae were more prevalent in the cryptogenic. Most of liver abscesses are single and right-sides in cryptogenic group.In both groups a high mortality was found because of antibiotic therapy alone.
