The Study Of Molecular Margin Of TSCC By Using MRI Defined The Tumor's Border

Objective:Exploring the molecular margin of TSCC to offer messages for doctors to induce the topo-recurrence rate of TSCC.Material and Method:Selecting 12 cases of medium-high differentiated TSCC, define the tumor's front border,inner border and rear border in the T2W1 and the picture strengthened by using Gd-DTPA according the signal intensity, then locate and compare the borders described by MRI and the borders defined by eyes and finger palpation on the tongue. After the surgery test the positive expressions of PCNA,COX-2,iNOS in the tissue of inside of tumor,0-2mm,5-7mm,10-12mm,15-17mm outside the tumor's border by immunohistochemical method, and observe the epithelial situation of the change on the cell lever by HE technique.Results:â‘ The average distance between the border of MRI and the border defined by eyes and fingers is 3.16mm.In the ront border,inner border and rear border the average distance are:1.78mm,2.44mm,5.28mm.There have significant difference between the distance in the rear border and the distance in the front border and the inner border.â‘¡No cancer cell was found in the tissue of 5-7mm outside of the MRI border, there have significant difference of situation of epithelial atypical hyperplasia between the tissue of MRI border and 15-17mm outside of the MRI border(P<0.05).â‘¢The positive expressions rate of PCNA in the tissue of cancer,0-2mm,5-7mm,10-12mm,15-17mm outside of the MRI border are:85.46%,63.79%,50.51%,42.25%,11.61%.There was no significant difference between the tissue of cancer.0-2mm,5-7mm,10-12mm outside of the MRI border, but have significant difference between the tissue of 15-17mm outside of the MRI border and the cancer inside and the MRI border (P<0.05). The positive expressions rate of COX-2 in the tissue of cancer.0-2mm,5-7mm,10-12mm,15-17mm outside of the MRI border are:69.87%,77.13%,63.72%,48.60%,20.52%.There was no significant difference between the tissue of cancer. 0-2mm.5-7mm.10-12mm outside of the MRI border, but have significant difference between the tissue of 15-17mm outside of the MRI border and the cancer inside (P<0.05).The positive expressions rate of iNOS in the tissue of cancer,0-2mm,5-7mm,10-12mm,15-17mm outside of the MRI border are:80.62%,81.26%,67.48%,50.90%,24.86%.There was no significant difference between the tissue of cancer. 0-2mm,5-7mm,10-12mm outside of the MRI border, but have significant difference between the tissue of 15-17mm outside of the MRI border and the cancer inside (P<0.05).Conclusion:â‘ The border of TSCC defined by MRI maybe exacter than the border defined by eyes and fingers on the tongue.â‘¡5-7mm outside of the MRI border maybe the cytological border of TSCC. â‘¢15-17mm outside of the MRI border maybe the molecular margin of TSCC.
