Objective As an advanced glycation end products (AGEs), capable of binding receptor for advanced glycation end products (RAGE), S100B plays an important role in the development of chronic diabetes complications; it also has vital effects on cell proliferation, differentiation, muscle contraction, gene expression and apoptosis through calcium ions signal transduction pathways. The current study was aimed (1) to understand the expression level of S100B mRNA and protein in the bladder detrusor of patients with benign prostatic hyperplasia (BPH) combined type 2 diabetes mellitus (T2DM), (2) to probe into the possible mechanism of bladder outflow obstruction (BOO) detrusor nerve damage about diabetic peripheral neuropathy in patients with BPH and T2DM, (3) to develop potential evidence-based clinical indicators of BOO detrusor, especially the parameters showing changes of BOO detrusor in patient with BPH and T2DM, (4) to provide experimental evidence for better treatment timing and preventing measures.Methods A total of 20 patients with BPH who were admitted to Haikou Hospital of Central South University from February 2009 to February 2010 were enrolled in this study. They were all diagnosed with BPH according to urinary ultrasonography, digital rectal examination and postoperative histopathologic examination. Urodynamic study showed bladder outlet obstruction and the contraction force of detrusor was not significantly weakened in all cases. Six patients were complicated with T2DM (experimental group), and the left 14 patients showed normal range of blood sugar (control group). All the patients underwent suprapubic prostatectomy, in which process, a strip tissue of front wall of bladder at size of 1.5cm×1.0cm×1.0cm was obtained from each patient. Use real-time quantitative polymerase chain reaction to evaluate the relative expression of S100B mRNA and western blot method to detect the relative expression of S100B protein in these tissues.Results1. The relatively expression of S100B mRNA in bladder detrusor of patients with BPH and T2DM was 0.399±0.880; in patients with simple BPH, it was 0.102±0.234, not statistically significant differences between the expression (P>0.05). 2. The relative expression of S100B protein in bladder detrusor of patients with BPH and T2DM was 1.21±0.42; in patients with BPH, it was 0.92±0.75, with no significant difference between the two (P>0.05).Conclusions1. When urodynamic report of patients with BPH and T2DM shows no obvious weakness of bladder contraction force, the relative expression of S100B mRNA and protein in bladder detrusor won't be significant different from that in patients with simple BPH.2. When there is no signs of diabetes cystopathy for patients with BPH and T2DM, treatment plan can be as same as that for simple BPH, while except surgery, diabetes education should be strengthened and blood sugar should be controlled actively in order to prolong the effect to the bladder detrusor caused by continues high blood sugar.
