Study Of The Relationship Between The Concentration Of Plasma Lipocalin-2 And Common Carotid Artery Intima-medial Thickness In Newly Diagnosed Type 2 Diabetic Patients

Objective:To investigate the relationship between the concentration of plasma lipocalin-2 and common carotid artery intima-medial thickness in newly diagnosed type 2 diabetic patients.Method:In this study,166 newly diagnosed type 2 diabetic patients (age 35-70, duration≤1 year) and 65 healthy people were recruited. Common carotid artery intima-medial thickness was detected by high-resolution ultrasonography, plasma concentration of lipocalin-2 was measured by enzyme-linked immunosorbent assay. Firstly, metabolic indexes,plasma concentration of lipocalin-2 were compared between normal people and type 2 diabetics group. Then, according to the plasma concentration of lipocalin-2,type 2 diabetic patients were divided into three groups:lowest concentration lipocalin-2 group,medial concentration lipocalin-2 group,highest concentration lipocalin-2 group. Clinical characteristics,metabolic indexes,common carotid artery were compared among these three groups.Basing on the carotid artery intima-medial thickness, these patients were also divided into three groups:lowest CCA-IMT group,medial CCA-IMT group,highest CCA-IMT group. Clinical characteristics,metabolic indexes,plasma concentration of lipocalin-2 were compared among these three groups. The relationship between concentration of plasma lipocalin-2 and common carotid artery intima-medial thickness was analyzed.Result:1)type 2 diabetics' age,BMI,We,WHR,SBP,plasma concentration of lipocalin-2,FBG,2hPG,FINS,HOMA-IR,TC,TG were significantly higher than the healthy group, P<0.05;adjusting for age and sex, the indexes mentioned above were still significantly higher in the type 2 diabetics group, P<0.05.2),According to the three tilter of plasma concentration of lipocalin-2,comparing these three groups'common clinical features, the highest concentration group's BMI,Wc,WHR,DBP were significantly higher than the lowest concentration group, P<0.05;while other indexes such as age,sex,history of smoking were no significant statistical differences among the groups. As for these three groups'metabolic indexes,the highest concentration group's HbAlc,FINS,HOMA-IR,TG were significantly higher than the lowest concentration group, however, there were no significant statistical differences in FBS,TC,HDL-C,LDL-C.Comparing these three groups'CCA-IMT, CCA-IMT increases along with the elevated concentration of plasma LCN2,the highest concentration group's CCA-IMT was significantly higher than the lowest concentration group, P=0.026.3),According to the three tilter of CCA-IMT,comparing these three groups'common clinical features, highest CCA-IMT group'BMI, WC.WHR were significantly higher than the lowest CCA-IMT group, P<0.05;either highest CCA-IMT group or medial CCA-IMT group, its DBP was significantly higher than the lowest CCA-IMT group, P<0.05; except for history of smoking, other indexes such as sex. history of drinking were no significant statistical differences among the groups. As for these three groups'metabolic indexes,the highest CCA-IMT group's HbAlc,FINS,HOMA-IR.TG were significantly higher than the lowest CCA-IMT group, P<0.05, while the medial CCA-IMT group's FBS. HbAlc were significantly higher than the lowest CCA-IMT group, P<0.05.There was significant statistical difference among these three group's plasma concentration of LCN2, P=0.000.Either the highest CCA-IMT group or medial CCA-IMT group, its plasma concentration of LCN2 was significant higher than that of the lowest group.4).In the linear correlation analysis, concentration of plasma LCN2 were positively correlated(P<0.05)with CCA-IMT.BMI,WC,WHR,DBP.HbAlc,FINS,HOMA-IR.TG; CCA-IMT were positively correlated (P<0.05)with concentration of plasma LCN2.age.BMI. HbAlc.In the multi-variables partial correlation analysis, adjusting for age.sex..BMI,WC,WHR,DBP,HbAlc,FINS,HOMA-IR,TG,LDL-C,the positive relationship between circulating LCN2 and CCA-IMT was still observed(r=0.197,P=0.007)。5),Multiply linear regression analysis demonstrated that circulating LCN2,age,BMI,LDL-C contributed to elevation of CCA-IMT in newly diagnosed type 2 diabetic patients.Conclusion:1)plasma concentration of LCN2 was much higher in newly diagnosed type 2 diabetic patients comparing to healthy people.2)plasma concentration of LCN2 was positively correlated with CCA-IMT in newly diagnosed type 2 diabetic patients.3)elevation of plasma concentration of LCN2 was probably one of the independent risk factors contribute to the elevation of CCA-IMT in newly diagnosed type 2 diabetic patients.
