308nm Excimer laser had performed its good and quick efficacy advantage since then it was applied in therapying vitiligo, but clinically to evaluate its potency still adopted the traditional way of reckoning roughly with naked eyes. This evaluation method is lack of science and objectivity. It's not only difficult to make patients convince,but also impact of Academic Exchange. At the same time,it can't sufficiently reflect patients'psychology and mental.Therefore, traditional evaluation ways were not able to meet the requirements of the modern biological-psychological-social medical model.Area of quantitative is key to clinical accurate evaluation of vitiligo or not. The current evaluation method depends on the doctors'experience on the measure the area of vitiligo and evaluate the recover-color rate.The method is only applied to the measure of the regular area,but many are irregular and have different shape pigmentum spot.The efficiency and reliability of method is very poor,and it is so easily influenced by the valuators'subjectivity that the results have the flaw of rough estimate and individual different juadgements.This method can not objectively analyzed and compared the effect assessment.Furthermore, the popular laser treatment costs are often associated with the size of white spot and patients be required multiple exposure in the treatment. A clinical assessment also should be divided. Therefore, a rapid, objective, accurate, reproducible objective evaluation method to replace the naked eye estimate is urgent need.With the development of the computer image processing technology, computer image analysis software has been able to achieve segmentation of the target image from the background image, and can give the features parameters of target image as needed. Therefore, we can use image processing software to extract the vitiligo lesions profile treated by excimer laser, calculate white area and the complex of color rate, and thus achieve the objective clinical evaluation of 308nm excimer laser for vitiligo.In order to improve the treatment efficacy of excimer laser for vitiligo evaluation system, reflect the objectivity,science and People-oriented thinking of Clinical evaluation, this study will apply computer image analysis software to clinical evaluation of the treatment efficacy of 308nm excimer laser for vitiligo. Further,we have researched this approach on the quality of life in patients with vitiligo. From the perspective of patients shows that the effectiveness of treatment or inadequate.Objective:1,To assess the effect of using image analysis software in extracting the vitiligo lesions profile treated by excimer laser, calculate white area and the complex of color rate, establish the best image analysis method for the treatment of 308nm excimer laser.2,To assess rhe clinical effect of 308nm excimer laser treatment for vitiligo using the best method of image analysis, analysis the factors of influencing efficacyl. At the same time to evaluate the impact of excimer laser on the quality of life in patients through investigating the quality of life of patients Method:1,Three dermatologists analyze the lesion area and re-color quantitative of the pre-treatment and after treatment using the method A (combine angel image analysis software and the photoshop)and B (angel image analysis software only) respectively. Spss 13.0 software was used to analysed the datas.2,We investigated patients'QOL face-to-face before treatment,after 1,2 and 3 treatments using DLQI, evaluated the therapeutic effect of 308nm excimer laser for vitiligo by Image analysis software method, investigated the factors of influencing efficacy and quality of life.Results:1,A method used as the assessment tool is better than B2,Efficiency evaluation and factors influencing efficacy1) The differences of recover-color rate among various course of treatmentAfter the 1st,2nd and 3rd course of treatment, the recover-color rate was35.85%±1.43%,51.59%±1.96%and 61.99%±2.15%,respectively. There was statistical significance among the differences of the 3 time points on whole comparing repeat factors(P<0.001). There was statistical significance comparing each other before and after treatment(P<0.001).2) The Comparison of clinical results among various course of treatmentThe mean rank of various course of treatments'recover-color results was 197.79, 152.34 and 105.87, respectively. The rate of getting better was reducing gradually with the increasing of course of treatments,but the rate of excellence and effectiveness was increasing gradually.3) The impact of Gender, age, Stage of the disease and course of disease on recover-color rate These results suggested that:①stage and course of disease have significant effect on recover-color rate among the normal data②there was negative correlation between recover-color and course of disease in the context of the same number of treatments and stage. The shorter the course of disease is,the higher recover-rate is.3,Treatment on quality of life of patients1) Values of DLQI before treatment and after various treatmentThe total values of DLQI was decreased gradually with treatment on. Before the treatment, the value of DLQI was 10.19±2.28; after the 1st,2nd and 3rd treatment,the value was 9.06±2.42,6.79±2.03 and 5.80±2.23,respectively. reduplicated factor was 4 and the difference of time level had statistical sense.(F=134.77,P<0.001).2) The effect of different recover-color level on evaluating scores of DLQI after three course of treatmentComparing the DLQI scores of various recover-color level after 3 course of treatment, Analysis of covariance was used to test wheather there was difference between the scores of DLQI,we found that there was significant difference between DLQI scores of various recover-color affected by the balance covariates(age,course of disease and the scores of DLQI)Conclusion:1) Image analysis software evaluation method can record the changes of recover-color rate continuously, the results are more objective and reliable than which by traditional Evaluation method.2) The main factors are stage of the disease and the course which can affect the re-color rate in general information.3) 308nm excimer laser treatment can not only improve the skin lesions but also effectively improve the patients'quality of life.
