The Relationship Between Hp Polymorphism And Serum Level With Agvhd In Patients Before And After Hsct

[objective] Detecting Hp phenotype and expression pre-and post-HSCT, to evaluate whether it associated with the development of aGVHD.[Content]1. To compare the distribution of Hp phenotype in aGVHD-,aGVHD+and normal controls.2. To compare the distribution of Hp phenotype in different grade of aGVHD.3. The relationship between HP phenotype and MTC.4. To clarify the pretransplant risk factors for aGVHD, we performed a logistic regression analysis.5. To compare the general level of Hp in aGVHD-and aGVHD+.6. To compare Hp expression in aGVHD+(the occurrence day), fever set and normal controls. To compare the Hp expression in different grade of aGVHD.[Methods]1. HSCT patients were divided into two group,aGVHD-and aGVHD+. And 100 cases of normal controls,50 males and 50 females.2. Detecting Hp phenotype with western-blot, and use PCR which we can't make sure.3. Measuring the serum Hp level with indirect ELISA.4. To statistically analyze the relation between the Hp phenotype distribution and serum level with aGVHD using SARS.[Result]1. The distribution of Hp among normal controls, aGVHD-and aGVHD+is not significant different.2. Patients withⅡ-ⅣaGVHD has more Hp2-2 than Hp2-1.3. Patients with Hp2-2 were at risk of MTC than other phenotype.(P<0.05)4. In logistic regression analysis, the results showed that unrelated donor, HLA-mismatch were associated with aGVHD development. (P<0.05)5. Results of patients serum Hp level in each team:the average value of aGVHD+ group was 5.858±1.184μg/μl, notably higher than the fever group and normal group(average value were5.234±0.97μg/μl and 1.135±0.571μg/μl respectively),was statistically significant.(P<0.001) And in each degrade of aGVHD, the difference was statistically significant. (P<0.001)[Conclusion] Results from experiments showed that the Hp polymorphism had no relationship with the occurrence of aGVHD. Logistic regression analysis showed that recipients age, unrelated donor were associated with aGVHD development. While Patients with gradeⅡ-ⅣaGVHD has more Hp2-2 than Hp2-1, it suggested that patients with Hp2-2 were inclined to develop serious aGVHD. And we can also conclude that patients with Hp2-2 are at risk of MTC, including pneumonia, VOD, aGVHD, ESL. From the change of Hp expression of view, that Hp level increased significantly when aGVHD occurred, and decreased when the disease was control. And the Hp expression was proportional to the severity of the aGVHD, the grade of aGVHD was significantly related to a high level of Hp expression. Preliminary results indicated that Hp polymorphism and expression had some relation with the aGVHD, and may be of value in diagnosis and judging the severity of the disease.
