Purpose:To evaluate the neuroprotection effect of Gross Saponin of Tribulus Terrestris(GSTT)on patients with intraocular pressure controlled-glaucoma.Methods:A prospective,randomized,double-masked,Positive control clinical trial was conducted.45patients(eyes)with moderate and late glaucoma were enrolled and randomly assigned to GSTT group and EBHM group which were gender,stage,intraocular pressure,and age-matched. Follow-up visits were scheduled every 2 months The following tests were performed at the beginning and at every visit: corrected visual acuity,IOP,MD(mean deviation),average retinal nerve fiber layer thickness (RNFL).Hemorheolog and color Doppler imaging system (CDI)were used to assesse the effectiveness of on the ocular blood at the beginning and final visit.Results There were no significant change (P>0.05) in corrected visual acuity,IOP,visual field, average retinal nerve fiber layer thickness (RNFL)for GSTT and EBHM groups during 6 months. All the MD change of GSTT group were significant (P<0.05)compared before and after treatment,except the first 2 months (P>0.05).The MD change of EBHM group were significant(P<0.05) in 2 months,4 months,6 months.The MD absolute value of GSTT had a decrease of 3.14 dB after treatment for 6 months,and it in EBHM group was 3.23dB. There was no significant difference between the two groups during the same period of the MD (P>0.05).The improvement rate of visual field of GSTT Group in the Second month,fourth month and sixth month were 11%,25%,35%, while EBHM group were 12%,23%,33%(P> 0.05).Both the blood rheology indicators of two group patients have varying degrees of improvement after 6 months treatment.The plasma viscosity, fibrinogen,low and high shear blood specific viscosity were significantly lower (P<0.01)in GSTT group after treatment There were a statistical significance difference(P<0.05) for fibrinogen and a significantly difference(P<0.01)for plasma viscosity in EBHM group after treatment.Conclusion:GSTT could improve visual field in patients with IOP controlled-glaucoma.GSTT has less risk for the glaucomatous optic neuropathy ant]greater effect on reducing the visual field defects. GSTT therapy shows favorable effect on patients with moderate and late glaucoma.
